Wang Maohong’s Experiential Study On Treating Chronic Kidney Disease With Kidney Main Gasification Theory

Research Background:In patients with chronic kidney disease are clinical frequently-occurring disease,according to the clinical manifestations of the disease belongs to traditional Chinese medicine can be turbid urine,bloody urine,kidney wind,edema,lumbago,kidney disease such as fatigue,difficulty in urination,block and repulsion,marking its basic pathogenesis is the virtual to real,are empty as this,with spleen and kidney deficiency,dampness and turbidity stasis toxin as standard.However,at present,the pathogenesis of deficiency,dampness,stasis and toxicity are mostly divided into different categories in clinical practice,and they cannot be effectively integrated.There is still a certain gap between them and the holistic view advocated by traditional Chinese medicine classics.Since the Huangdi Neijing,ancient doctors have elaborated more on the gasification of kidney,and gradually formed the theory of the main gasification of kidney.Renal master gasification is a high generalization of renal physiological function.Kidney is the house of fire and water,Yang in Yin for the qi,the movement of kidney essence is also manifested as gasification.But at present,the understanding of the theory of kidney main gasification is not perfect,the understanding of the relationship between chronic kidney disease and the theory of kidney main gasification is still shallow,and the application of the theory of kidney main gasification in clinical treatment of chronic kidney disease has not been deeply explored and applied.By inheriting Professor Pizhiheng’s academic thought of "kidney main gasification" and digging classics,my tutor believes that chronic kidney disease is closely related to kidney main gasification after more than 30 years of clinical practice.Therefore,to further study the theory of kidney main gasification,to explore the overall relationship between deficiency,dampness,stasis and toxicity of chronic kidney disease starting from the gasification of kidney,and to sort out the clinical experience of applying the theory of kidney main gasification in chronic kidney disease,is of certain significance for expanding the treatment of chronic kidney disease and improving the clinical efficacy of this kind of disease.Research objectives:1.To explore the theory of kidney main gasification and the theoretical basis of treating chronic kidney disease.2.To explore the relationship between the theory of kidney main gasification and deficiency,dampness,stasis and toxicity of chronic kidney disease.3.To summarize the clinical experience of supervisor Wang Maohong in treating chronic kidney disease with the theory of kidney main gasification.Research methods:1.Through sorting out,summarizing and analyzing the concepts of gasification and kidney main gasification,the fundamental of viscera gasification and the relationship between kidney and other viscera gasification in ancient books such as Huangdi Neijing,Treatise on Cold Injuries,Jing Yue,Bi Hua Medical Mirror,etc.,the theory of kidney main gasification and its theoretical basis for the treatment of chronic kidney disease are discussed.2.Through induction,analysis and summary,explore the relationship between the theory of kidney main gasification and the deficiency,dampness,stasis and toxicity of chronic kidney disease.3.Through searching and reading the papers of my tutor and fellow seniors,combined with the patients treated by my tutor Pingsu in the outpatient department and inpatient department,I sorted them into medical records,analyzed the general treatment methods of my tutor’s application of the kidney master theory in the treatment of chronic kidney disease,and summarized the common witness types and prescriptions of my tutor on the early,middle and late stage of chronic kidney disease.4.Through collecting the cases of chronic kidney disease treated and treated by the tutor,according to the laboratory examination and clinical characteristics of the patients,they were divided into early,middle and late stages,corresponding to the 1-3a,3b-4 and 5 stages of chronic kidney disease respectively.The above data were sorted out and analyzed,and the syndrome types and prescription frequency of different periods were summarized.The research supervisor applied the theory of kidney main gasification to discuss the syndrome and treatment law of chronic kidney disease.Research results:1.Kidney main gasification actually summarizes the overall function of the kidney,is the basis of whole body gasification.Dysregulation of renal main gasification is the main pathogenesis of chronic kidney disease,including under-gasification of kidney and adverse gasification of kidney,which are closely related to the pathogenesis of "deficiency,dampness,stasis and toxicity" of chronic kidney disease.Therefore,it is the key to adjust and restore the function of renal gasification in clinical treatment of the disease.2.According to the theory of kidney master gasification,there are four main treatment methods for treating chronic kidney disease: taking kidney as the base,regulating five viscera to regasify;Through evil Fuzheng,to regasify;Adjust the lifting,reach the air machine to regasification;Focus on daily care for regasification.And according to the above treatment methods summed up the early,middle and late chronic kidney disease often witness type and prescription drugs.3.Through statistical analysis of the chronic kidney disease cases collected from the outpatient department,it is found that the basic rules of the tutor’s diagnosis and treatment of chronic kidney disease in clinic are as follows: Early with kidney deficiency,fu evil outgoing is given priority to,the evil to Yin,the outgoing in less Yang less and less evil wind water in v type,outgoing in shaoyin two certificate,accounted for more than 48.7%,with ephedra asarum lateral root of aconite decoction on selected clinical use most frequently,accounted for 45%,in this period to use drugs for most dangshen,astragalus root,monkshood,Gordon euryale seed;In terms of prescription selection,Wendun Decoction and Guizhi poria Wan were the main components,accounting for 47%.In this stage,although the main components were evil substance,in clinical prescription selection,such as promoting blood circulation and removing blood stasis,or removing dampness and removing turbidity,regulating the rise and fall and reaching Qi organs,the kidney’s gasification was unfavorable after treatment,but the disease itself was based on deficiency.Therefore,kidney tonifying drugs should be used to restore the gasification of kidney,so the frequency of use of three drugs in the middle period is very high.In the late stage of chronic kidney disease,due to the close relationship between the kidney and the spleen,the late congenital has been abandoned,many patients have entered the stage of kidney replacement therapy,and only 16 cases have been collected.Therefore,it is necessary to strengthen the spleen and stomach.The most common syndrome type is middle-deficiency and turbidness,accounting for50%,Wuzhuyu Decoction,Xiaobanxia decoction and rhubarb liquorice decoction are used more frequently.The highest frequency of faxia and Wuzhuyu was 87.5%.Rhubarb and dried ginger accounted for 70.8%.4.The common drug pairs for the treatment of chronic kidney disease by the tutor were summarized as: rhubarb with aconite,ephedra with asarum,Coptidis with Su leaf,ume plum with cicada slua,ejiao with talc,etc.Conclusion:1.The theory of kidney main gasification originated from the Huangdi Neijing and was developed in later generations,but it was not isolated into articles,but scattered in relevant discussions.Through systematic arrangement,it is found that the theory of kidney main gasification can realize the whole function of kidney in a more systematic and comprehensive way,which is worthy of further study to broaden the clinical thinking.2.By applying the theory of kidney master gasification to the treatment of chronic kidney disease,the four major pathogenesis of chronic kidney disease including deficiency,dampness,stasis and toxicity can be linked as a whole from the start of kidney gasification.Therefore,adjusting and restoring kidney gasification has certain clinical significance in the treatment of chronic kidney disease.
