Objective:To observe the distribution pattern of force sensitive acupoints on the body surface of patients with Cervical shoulder Myofascial pain syndrome(CMPS),to explore the clinical effectiveness and efficacy differences between mechanosensitive acupuncture and conventional acupuncture in the treatment of cervical shoulder myofascial pain syndrome,and to provide new clinical ideas for the treatment of cervical and shoulder myofascial pain syndrome with acupuncture.Methods:1.Trial 1: 100 patients with CMPS were included for mechanosensitive acupuncture point exploration.The locations of the mechanosensitive acupuncture points explored were recorded on the sensitized acupuncture point exploration record sheet,the pain threshold of the mechanosensitive acupuncture points of the patients was measured using a hand-held pain meter and recorded,and the distribution pattern of force-sensitive acupuncture points of CMPS patients was statistically analyzed.2.Trial 2: Among the 70 patients with 5 or more mechanosensitive acupoints detected,60 were randomly selected for the study and randomly divided into a test group of 30 patients and a control group of 30 patients by the random number table method.Treatment method:(1)Experimental group(mechanosensitive acupuncture group):5 mechanosensitive acupuncture points(the first 5 were selected according to the measured pressure pain threshold in descending order)were acupunctured,and the acupuncture method was applied in a flat tonifying and flat diarrhoeic manner,and the acupuncture points were retained for 30 min after obtaining Qi.(2)Control group(conventional acupuncture group): Tianzhu acupuncture point,Jian jin acupuncture point,Jian zhen acupuncture point,Geshu acupuncture point and Yang lingquan acupuncture point(all acupuncture points are taken bilaterally),the acupuncture method is the same as that of the mechanosensitive acupuncture point group.The treatment was carried out once a day,7 times for 1 course of treatment,and 1 course of treatment was observed.The simplified Mc Gill Pain Scale and the NPQ Neck Pain Scale were completed before,after and at the follow-up period to evaluate and analyse the improvement of the pain symptoms and the efficacy of the two treatments in the near and long term.Results:1.Trial 1 showed that in 100 patients with cervical and shoulder myofascial pain syndrome,a total of 532 mechanosensitive acupoints were explored,with an average of 5.32 mechanosensitive acupoints per patient and 70 patients with 5 or more mechanosensitive acupoints.The top ten mechanosensitive acupoints in patients with cervical and shoulder myofascial pain syndrome were: Shoulder Well,Tianjiao,Hand Sanli,Baihui,Jueyin Yu,Fengchi,Neck Bailao,Bingfeng,Xiao hai1 inch,and Houxi;the top five meridians with the highest number of mechanosensitive acupoints were: Hand Sun Small Intestine Meridian,Foot Sun Bladder Meridian,Foot Shaoyang Gallbladder Meridian,Hand Yang ming Large Intestine Meridian,and Governor Meridian.The most frequent areas for mechanosensitive acupoints are the neck and shoulders,followed by the upper limbs,head and face,chest and back,front of chest and lower limbs.2.Trial 2:(1)General information: the differences in age,gender and duration of disease data between the two groups before treatment were tested statistically,all P>0.05,suggesting no statistical significance,which can be compared and analysed.(2)Recent efficacy: After one course of treatment,the Mc Gill score and NPQ score of both the test group and the control group were lower than those before treatment,and the differences between the scores of the two groups before and after treatment were statistically significant by statistical tests(both P<0.01),indicating that both mechanosensitive acupuncture and conventional acupuncture could improve the pain situation of CMPS patients.When comparing the post-treatment scores of the two groups,the test group showed a more significant decrease in scores than the control group,with a statistically significant difference(P<0.01),indicating that force sensitive acupuncture treatment was more effective than conventional acupuncture treatment in improving pain.In combination with the overall efficacy,the test group(mechanosensitive acupuncture group)cured 4 cases,showed 7 cases,effective 16 cases and invalid 2 cases,with a total effective rate of 93.1%;the control group cured 2 cases,showed 3 cases,effective 15 cases and invalid 9 cases,with a total effective rate of 69.0%,the total effective rate of the test group was higher than the total effective rate of the test group(P<0.01),indicating that the recent efficacy of mechanosensitive acupuncture treatment was better than that of conventional acupuncture(3)Long-term efficacy: During the follow-up period(after 1 month),the Mc Gill score and NPQ score of both the test group and the control group were lower than those before the treatment,which were statistically tested(both P<0.01),suggesting that the differences in the data were statistically significant,indicating that both mechanosensitive acupuncture and conventional acupuncture have certain long-term efficacy.When the two groups were compared for follow-up period scores,the test group showed a more significant decrease in scores than the control group,with a statistically significant difference(P <0.01),indicating that the long-term clinical efficacy of mechanosensitive acupuncture was also superior to that of conventional acupuncture.Conclusion:1.The mechanosensitive acupoints in patients with cervical and shoulder myofascial pain syndrome were distributed according to the attributed meridians,with the highest correlation to the hand sun and small intestine meridians;according to the distribution of sites,the most distributed site was the cervical and shoulder region,followed by the upper limb region.2.Mechanosensitive acupuncture treatment can effectively improve the pain symptoms of CMPS patients,and its clinical efficacy is better than that of conventional acupuncture treatment,and its long-term efficacy is also better than that of conventional acupuncture treatment in follow-up.
