| GPO is a drug group procurement model originated in the United States.In 2016,pilot work was carried out in Shanghai and Shenzhen,and important results were achieved.In recent years,with the strong promotion of the national medical and health reform,a variety of drug procurement models have also emerged,such as centralized and volume procurement organized by the state,and regional alliance procurement led by local governments,which have played an important role in saving drug procurement costs and improving people’s well-being.As an important drug procurement model,GPO has been recognized by the drug procurement market due to its high degree of marketization and flexible operation,while many other procurement models have demonstrated their skills.However,unlike national or local led group procurement models,joining a GPO organization requires medical institutions to pay a certain amount of membership fees,while charging a certain proportion of contract management fees to drug suppliers,which to some extent weakens the willingness of some medical institutions or drug suppliers to join the GPO organization or engage in cooperation.Therefore,GPO organizations need to broaden their service functions,attract more medical institutions to join GPO organizations,and promote cooperation with drug suppliers.GPO revenue compensation is one of the important service functions of GPO organizations.After deducting costs,the remaining portion of GPO revenue will be returned to member hospitals of GPO organizations in the form of cost compensation,which can further reduce the procurement costs of member units,encourage more medical institutions to choose to join GPO organizations,and attract more drug suppliers to participate in cooperation.However,this also means a loss of some of the interests of the GPO organization,so the GPO organization needs to consider comprehensive consideration of future development and realistic benefits.This article comprehensively uses theories such as drug supply chain,revenue compensation,interpretative structural models,and evolutionary game theory to study the drug procurement strategy of medical institutions considering GPO revenue compensation,with the research objects of GPO organizations,medical institutions,and drug suppliers in the drug group procurement supply chain.Firstly,use literature research method to sort out the influencing factors of GPO revenue compensation,and invite experts to demonstrate them.On this basis,establish an explanatory structural model of the influencing factors of GPO revenue compensation,and conduct a hierarchical analysis,dependency driving force analysis of the influencing factors;Build a tripartite game model between GPO organizations,medical institutions,and drug suppliers,analyze the impact of different entities on evolutionary stability,and use Matlab to conduct simulation to analyze the impact of single parameter changes and multiple parameter changes on the evolutionary game system.Finally,based on the above analysis,propose an optimization strategy to promote medical institutions to choose drug group procurement considering GPO revenue compensation conditions.Based on the above research,the following conclusions are drawn:(1)There are 17 factors affecting GPO revenue compensation,which can be divided into four levels and have four driving factors.(2)In a tripartite game model that considers GPO revenue compensation among GPO organizations,medical institutions,and drug suppliers,when GPO organizations choose "compensation",medical institutions choose "join",and drug suppliers choose "participate",the tripartite main body game can reach a stable state.(3)When the cost of revenue compensation provided by GPO organizations is small,the probability of GPO organizations choosing "compensation" increases;Whether a GPO organization provides revenue compensation services depends more on the cost and benefits of providing revenue compensation services;The increase in external and direct benefits obtained by GPO organizations providing revenue compensation will promote the system to evolve to a stable point;Medical institutions providing revenue compensation services can enable the system to evolve to a stable state;When medical institutions pay more for independent procurement,they are more likely to choose to "join" GPO organizations;When the revenue of drug suppliers from selling drugs at the original price is low,drug suppliers are more likely to choose to "participate" in group procurement;When drug suppliers participate in group procurement,their drug production can form a scale effect,and the higher their production cost savings,the more stable the system evolution results;The probability of medical institutions joining GPO organizations without revenue compensation has a significant impact on the stability of the evolutionary system.Changes in the revenue compensation obtained by medical institutions joining GPO organizations have a significant impact on the results of system evolution.(4)Considering GPO revenue compensation,optimizing the procurement and supply chain of drug groups can take multiple measures from the perspectives of government,GPO organizations,medical institutions,and drug suppliers to promote medical institutions to choose to join GPO organizations. |
