| Objectives1、To analyze the influencing factors of the effect of rehabilitation exercise for stroke hemiplegia patients at home,and to provide clinical basis for formulating corresponding rehabilitation exercise measures.2、To explore the influence of the "Internet +" H2 H rehabilitation management model on the motor function,compliance of rehabilitation exercises,activities of daily living and nursing satisfaction in the rehabilitation process of home stroke patients with hemiplegia,in order to standardize the home rehabilitation of stroke patients with hemiplegia management for reference.Methods1 、 Using the convenience sampling method,248 patients with stroke and hemiplegia who came to our hospital for follow-up visits from January 2021 to June2021 were selected as the research objects.Questionnaire surveys were carried out using the patient’s general information questionnaire,the Hamilton Depression Scale(HAMD),the Family Environment Scale(FES),the Modified Barthel Index Scale(MBI)and the Functional Activity Questionnaire(FAQ).To understand the current situation of home rehabilitation exercise and related influencing factors for stroke patients with hemiplegia,and analyze the correlation between the influencing factors of stroke home rehabilitation exercise and MBI and FAQ.Therefore,a rehabilitation management model based on "Internet +" was formulated.2 、 Select 80 stroke hemiplegia patients who came to our hospital from July2021 to June 2022 as research objects,and divide them into groups according to the date of admission.The odd number is the control group,and the even number is the observation group,40 cases in each group.The control group was given routine disease health education and discharge guidance,mainly including diet,medication,and physical exercise.When patients were discharged from the hospital,they were given stroke diet maps,physical exercise manuals,precautions for stroke recurrence prevention,and regular telephone follow-up.On the basis of the control group,the observation group implemented the "Internet +" H2 H rehabilitation management model for intervention.Mainly through the establishment of a "H2H" rehabilitation management team,a comprehensive functional rehabilitation exercise plan is formulated,and the whole process of patient management including health education,personalized guidance,follow-up reminders and follow-up is carried out relying on the rehabilitation management platform.Before the intervention,1 month after the intervention,and 3 months after the intervention,the general information questionnaire,the Fugl-Meyer Motor Function Assessment Scale(FMA)and the Stroke Rehabilitation Exercise Compliance Scale were used in this study.,Barthel Index Scale(BI)for questionnaire survey,and after 3 months of intervention,stroke specific quality of life scale(SS-QOL)and nursing satisfaction questionnaire were used Quality of life and satisfaction surveys,and evaluation of intervention effects.Results1 、 Univariate analysis showed that there were statistically significant differences in MBI scores among different age groups,home environment,history of hyperlipidemia,obesity,rehabilitation start time,and rehabilitation duration(P<0.05).There were statistically significant differences in the FAQ scores of patients with different occupations,monthly family income,and depression(P<0.05).The results of Pearson analysis showed that MBI and FAQ scores were significantly correlated with age(r=-0.584,r=-0.438),occupation(r =-0.235,r=-0.103),hyperlipidemia(r=-0.339,r=-0.136),obesity(r=-0.131,r=-0.304),depression(r=-0.573,r=-0.552),rehabilitation start time(r=-0.783,r=-0.599)was negatively correlated,MBI and FAQ scores were correlated with family monthly income(r=0.452,r=0.491),family environment score(r=0.662,r=0.724),rehabilitation duration(r=0.572,r=0.603)was positively correlated.Multiple linear regression analysis showed that age,monthly family income,family environment score,depression,rehabilitation start time,and rehabilitation duration were independent factors affecting the effect of rehabilitation exercise in stroke patients with hemiplegia.2、The analysis of the results of the intervention study showed that there was no significant difference in the scores of the baseline data,Fugl-Meyer motor function scale,stroke rehabilitation exercise compliance scale,and Barthel Index scale between the two groups(P>0.05).After 1 month and 3 months of intervention,the Fugl-Meyer motor function score,functional exercise compliance score and BI score of the two groups were higher than those before the intervention,and the observation group was significantly better than the control group(P<0.05).The results of repeated measures analysis of variance showed that the Fugl-Meyer motor function score,functional exercise compliance score and daily living activity score of the two groups of patients were significant in terms of between-group,within-group and interaction(P<0.05).After 3 months of intervention,the patients’ quality of life and nursing satisfaction were investigated using the stroke-specific quality of life scale(SS-QOL)and the Newcastle Satisfaction Scale,and the quality of life and total satisfaction of nursing in the observation group were significantly higher than those in the control group group,the difference was statistically significant(P<0.05).Conclusion1 、 The research results show that age,monthly family income,family environment score,depression,rehabilitation start time,and rehabilitation duration are all independent factors affecting the effect of rehabilitation exercise in stroke patients with hemiplegia.The effect of rehabilitation exercise was negatively correlated with age,occupation,hyperlipidemia,obesity,depression,and t rehabilitation start time,and the effect of rehabilitation exercise was positively correlated with monthly family income,family environment,and rehabilitation duration.2 、 Based on the "Internet +" H2 H rehabilitation management model,it can effectively improve the limb motor function of stroke patients with hemiplegia,improve the standardization of patients’ limb training movements,improve the effectiveness of patients’ home rehabilitation training,improve the
