Objectives: To investigate the relationship between intrapartual fever and perinatal neonatal diseases and the risk factors of neonatal diseases,in order to provide more evidence for early identification and intervention of perinatal neonatal diseases.Methods: Puerpera with intrapartum fever and their newborns in Inner Mongolia Maternal and Child Health Hospital from January 2020 to December 2022 were selected as the observation group,and puerpera with intrapartum fever and their newborns were randomly selected as the control group.The demographic and clinical data of the enrolled cases were collected,including maternal age,delivery mode,gestational age at delivery,maximum body temperature during labor,time from intrapartum fever to fetal delivery,epidural labor analgesia,premature rupture of membranes,amniotic fluid contamination grade,maternal peripheral blood white blood cell count,blood high-sensitivity C-reactive protein,procalcitonin and neonatal outcomes.The subjects were grouped according to whether the mother had intrapartum fever or not,and the intrapartum fever group was grouped according to the maximum temperature at labor,the duration of fever,and the incidence of neonatal diseases.Multivariate Logistic regression analysis was used to explore the effect of intrapartum fever temperature on neonatal diseases.Results: A total of 488 puerpera with intrapartum fever and 450 puerpera with intrapartum fever and their newborns were enrolled.In the intrapartual-fever group,the maternal age was28.81±2.42 years,and the gestational age at delivery was 39.78±1.21 weeks.The maternal age of the proletarian fever group was 28.82±2.41 years,and the gestational age at delivery was 39.59±1.12 weeks.The incidence of neonatal diseases in the intrapartum fever group was higher than that in the proletarian fever group(P<0.001).Compared with the non-fever group,the fever group had a significantly higher incidence rate of infection,pneumonia,wet lung,aspiration syndrome,pneumothorax,Apgar score <7 at 1 minute after birth,and intracranial hemorrhage(P<0.05).The incidence of neonatal diseases in the severe and moderate elevation groups was significantly higher than that in the mild elevation group(P<0.05).The severe-elevation group had a significantly higher incidence rate of Apgar score <7 at 1 minute after birth than the moderate-elevation and mild-elevation groups(P<0.05).The severe elevation of body temperature group had a significantly higher incidence rate of neonatal aspiration syndrome than the mild elevation group(P<0.05).Among the neonates in the fever group,the proportion of mild maternal body temperature elevation in the group with neonatal diseases was lower than that in the group without neonatal diseases,and the proportion of moderate and severe maternal body temperature elevation was higher than that in the group without neonatal diseases,and the difference in the degree of fever between the two groups was statistically significant(P=0.019).The proportion of Ⅲ degree amniotic fluid contamination in the group with neonatal diseases was higher than that in the group without neonatal diseases,and the difference was statistically significant(P<0.05).Logistic regression analysis with neonatal diseases as the outcome variable showed that the risk of neonatal diseases increased with the increase of intrapartural fever temperature(OR:2.47,95%CI:1.30-4.71,P=0.006).In the intrapartum fever group,the proportion of mothers with mild elevation of body temperature in the respiratory disease group was lower than that in the non-respiratory disease group,and the proportion of mothers with moderate elevation and severe elevation of body temperature was higher than that in the non-respiratory disease group.The difference in the degree of intrapartum fever between the two groups was statistically significant(P<0.001).The proportion of Ⅲ degree amniotic fluid contamination in the respiratory disease group was higher than that in the non-respiratory disease group,and the difference was statistically significant(P<0.05).Logistic regression analysis with neonatal respiratory diseases as the outcome variable showed that the risk of neonatal respiratory diseases increased with the increase of intrapartural fever temperature(OR:2.46,95%CI:1.34-4.53,P=0.004).Conclusions: Conclusions Intrapartum fever increases the incidence of neonatal diseases,infection,pneumonia,wet lung,aspiration syndrome,pneumothorax,Apgar score <7 at 1minute after birth and intracranial hemorrhage.With the increase of maternal fever temperature during delivery,the risk of neonatal diseases and neonatal respiratory diseases increased,and the incidence of neonatal 1-minute Apgar score <7 and neonatal aspiration syndrome increased.
