| Objective To analyze the efficiency and safety of endovascular therapy(EVT)for patients with minor stroke with large vessel occlusion(mLVO-AIS).Methods Patients with mLVO-AIS admitted to the Green Channel of Stroke in the Shanghai No.6 People’s Hospital from June 2016 to October 2022 were retrospectively included,who received emergency EVT as endovascular therapy group(EVT group),and patients who only received standard medical treatment in the same period were selected as the control group.General clinical data and parameters related to endovascular therapy were collected.The NIH stroke scale(NIHSS)was used to assess the degree of neurological defecation,and the primary outcome was the NIHSS score at seventh day(d7NIHSS)after treatment.d7NIHSS score decreased by≥3 points from baseline NIHSS or decreased directly to 0 points defined as effective.Secondary outcomes included successful revascularization rate,early neurological deterioration(END),symptomatic intracranial hemorrhage(s ICH),and mortality.Results A total of 63 patients were included,with a mean age of 67.2±14.4 years,24(38.1%)patients were effective.The effective rate in the EVT group was51.6%(16/31)and the effective rate in the control group was 25%(8/32)(X~2=4.729,P=0.030),the difference of effective rate was statistically significant(P<0.05).The baseline NIHSS of EVT group 5(3,5)was higher than the baseline NIHSS of control group 3.5(2,5),and the d7NIHSS of EVT group 1(0,3)was lower than the d7NIHSS of control group 2(1,5.75).1 case of END in the EVT group,8 cases of END in the control group(X~2=6.097,P=0.014),the difference of END was statistically significant(P<0.05).1 case of cerebral infarction hemorrhage transformation in the control group.The head CT examination 24 hours after operation showed no bleeding,no death during hospitalization,there was no difference in safety between the two groups.Baseline NIHSS of 24 patients operated with EVT was 5(3,5),24h NIHSS after EVT was 2(0.25,3.75),d7NIHSS after EVT was 1(0,2.75),Kruskal-Wallis H test(H=16.997,P<0.001),the difference of NIHSS was statistically significant(P<0.05).Conclusion Endovascular therapy in mLVO-AIS is safe and effective.Endovascular therapy has a better early efficiency in mLVO-AIS,with a lower risk of END and the risk of s ICH is not increased.Figure[2]Table[5]Reference[71]... |
