| Objective:This study mainly investigated the current situation and influencing factors of the self-perceived burden of cervical HPV infected persons,and developed intervention programs for patients with PRECDE-PROCEED model as the guidance framework,to explore the clinical effect of intervention programs on patients’ self-perceived burden,and to provide theoretical basis for optimizing the clinical outcome of patients.Methods:The first stage: Based on the current situation investigation,a total of 243 patients with cervical HPV infection who met the inclusion and exclusion criteria in the gynecological outpatient department of Ordos Central Hospital from March 2022 to July 2022 were selected by convenience sampling method.General data questionnaire,Self-perceived burden scale,Chronic disease management self-efficacy scale and Family support self-rating scale were used as research tools.The current situation of the patients was studied,and 3 questionnaires that did not meet the filling requirements were eliminated.The influencing factors and current situation of the self-perceived burden of 240 cervical HPV infected patients were analyzed,which provided theoretical support for the second stage of PRECDE-PROCEED model health education intervention.The second stage: Based on the similar experimental study,74 patients with cervical HPV infection in the gynecological outpatient department of Ordos Central Hospital from August 2022 to March 2023 were selected as the research objects.The subjects were grouped according to the random number table method,and 37 patients were included in each group.In order to prevent the interference of confounding factors,the intervention times of patients in the two groups were the same.The control group was given the department routine health education mode,while the intervention group was given the department routine health education based on the PRECDE-PROCEED model.Two people in the intervention group were absent from knowledge lectures for too many times,which was regarded as shedding cases;one person in the control group was absent from knowledge learning and one person filled in the questionnaire illogically.Finally,35 patients in the control group and the intervention group were infected with cervical HPV,respectively.Outcome measures: The scores of cervical HPV infected patients before enrollment,1 month and 3 months after intervention,Self-perceived burden,Chronic disease management self-efficacy and Family support were compared between the two groups.Results: 1.Survey results of current research stage(1)The self-perceived burden score of 240 patients with cervical HPV infection was25.96±10.08,which was at the medium level.The average score of each dimension from high to low was: physical burden,emotional burden,economic burden;33patients(13.8%)had severe SPB,54 patients(22.5%)had moderate SPB,91 patients(37.9%)had mild SPB,and 62 patients(25.8%)had no obvious SPB.(2)The total mean score of Family support of cervical HPV infection patients was10.39±2.53,belonging to the moderate level of support,121 patients(50.4%)had moderate level of family support,79 patients(32.9%)had high level of family support,and 40 patients(16.7%)had low level of family support.The total mean score of self-efficacy in patients with cervical HPV infection was 14.33±4.65.(3)The correlation analysis between the perceived burden of cervical HPV infection patients and family support and self-efficacy showed that the total score of the perceived burden of cervical HPV infection patients and the scores of each dimension were negatively correlated with Family support(r =-0.402 ~-0.328,P < 0.001).The perceived burden and dimensions of cervical HPV infection patients were negatively correlated with Self-efficacy(r =-0.743 ~-0.463,P < 0.001).(4)Multiple linear regression analysis of factors affecting the perceived burden of cervical HPV infection patients showed that: With SPB of cervical HPV infection patients as the dependent variable,multiple linear regression analysis was performed on seven variables including statistically significant education level,per capita family income,primary caregiver’s health status,working status,payment method of medical expenses,HPV infection status,HPV subtype,family support and self-efficacy in one-way ANOVA.The results showed that household per capita monthly income,HPV infection status,HPV subtypes,and working status were the main factors affecting the perceived burden of patients with cervical HPV infection.In summary,the regression equation can be obtained as follows: self-perceived burden score((?))=34.241-4.62 per capita monthly household income-1.597* HPV infection +2.173*HPV subtype-4.2* No job.2.Results of two experimental stages(1)Before the implementation of intervention measures,there was no statistical significance in the general information,self-perceived burden,family support and self-efficacy scores of patients with cervical HPV infection between the control group and the intervention group(P>0.05),they were comparable.(2)After 1 month and 3 months of Green mode health education intervention,the self-perceived burden score of the intervention group was reduced compared with the control group,and the difference was statistically significant at the same time point after intervention(P<0.05).(3)The family support of patients in the intervention group was better than that of patients in the control group 1 and 3 months after the Green model health education intervention,and the difference was statistically significant at the same time point after intervention(P<0.05).(4)The self-efficacy scores of patients in the intervention group were higher than those in the control group at 1 and 3 months after Green mode health education intervention,and the difference was statistically significant at the same time point after intervention(P<0.05).Conclusions:1.74.2% of patients with cervical HPV infection had self-perceived burden ranging from mild to severe.2.50.4% of patients with cervical HPV infection belonged to the middle level of Family support,and their Chronic disease management self-efficacy score was 14.33±4.65,which needed to be improved.3.The family per capita monthly income,the health status of the primary caregiver,HPV infection status,working status(sick leave or retirement,in-service work),occupation(technicians,businessmen,medical personnel)are the main factors affecting the Self-perceived burden of cervical HPV infection patients.4.Family per capita monthly income,HPV infection status,HPV subtypes,working status are the main factors affecting the self-perceived burden of cervical HPV infection patients.5.In this study,nursing intervention based on PRECDE-PROCEED model can improve the level of family support and self-efficacy of patients with cervical HPV infection,and effectively reduce the level of self-perceived burden of patients with cervical HPV infection.The intervention effect is positively correlated with the duration of intervention process,which plays a positive role in the prevention and treatment of HPV infection and is worthy of clinical promotion and application. |
