Objective: By studying the expression level of miR-218 and HMGB1 in the plasma of children with wheeze pneumonia,we investigated the role of miR-218 and HMGB 1 in the pathogenesis of wheeze pneumonia in infants.Methods:Selected 30 children of 6 months to 3 years and non-wheezing pneumonia from2021 of Inner Mongolia Autonomous Region from September 2021 to September 2022 as the experimental group,Thirty cases of healthy children during the same physical examination were in the control group,Quantitative real-time polymerase chain reaction(Quantitative Real-time PCR,By q PCR),enzyme-linked immunosorbent test(ELISA),detecting the plasma levels of miR-218 and HMGB 1 in children with wheezing pneumonia,non-wheezy pneumonia and normal children,Compare the expression levels between the two and analyze the correlation,ROC curves were used to analyze miR-218,HMGB 1,To assess its predictive value for wheeze pneumonia.Results:1.The plasma expression level of miR-218 in the acute phase of the wheezing group was significantly lower than that of the non-wheeze group and the control group.The plasma level of miR-218 in the non-wheeze group was significantly lower than that of the control group,which was statistically significant(P<0.05).The plasma level of HMGB1 in the acute stage was significantly higher in the wheeze group than in the non-wheeze group and the control group,and the acute stage HMGB1 level was significantly higher in the non-wheezing group,which showed statistically significant differences(P<0.05).2.There were no significant differences in convalescent plasma miR-218 and HMGB 1in the wheezing group compared with the control group(P> 0.05).3.In the wheezing group,plasma miR-218 was lower than the recovery period,and HMGB1 was higher than the recovery period,which showed statistically significant differences(P <0.05).In the non-wheeze group,the expression level of miR-218 in the acute plasma was lower than the recovery period,and the HMGB1 level was higher than the recovery period,which showed statistically significant differences(P<0.05).4.The plasma miR-218 expression level was significantly negatively correlated with HMGB1 in the acute period(r=-0.898,P<0.05),and the expression level of miR-218 was significantly negatively correlated with HMGB1 in the convalescent period(r=-0.638,P<0.05).In the non-wheezing group,the plasma miR-218 was significantly negative and HMGB 1(r=-0.685,P<0.05),and the convalescent plasma miR-218 was significantly negative and HMGB1(r=-0.817,P <0.05).5.To evaluate the predictive value of miR-218 and HMGB1 for wheezing bronchopneumonia,The ROC curve was selected for the analysis,Through the ROC curve analysis of two indicators,miR-218 and HMGB1,That the area under the curve of miR-218(AUC)is 0.800,A sensitivity of 80.0%,The atality was 73.3%,The corresponding cutoff value is 1.254;The HMGB1 AUC is 0.842,A sensitivity of 70.0%,The atality was 86.7%,The corresponding cutoff value was 28.825 ng / ml.Both AUC was greater than 0.7,indicating that miR-218 and HMGB 1 had some predictive value for wheezing pneumonia.6.There was no significant difference between age and sex in the wheezing group,non-wheezing group and control group.In the wheezing group,the hospital stay was longer than the non-wheezing group,the percutaneous arterial oxygen saturation(SPO 2)was lower than the non-wheezing group,statistically significant(P<0.05),and the eosinophil count(EOS)and absolute neutrophil value(NEUT)were higher than the non-wheezing group(P<0.05).There was no significant difference in the wheezing group in fever time,leukocyte count(WBC),absolute lymphocyte value(LY),platelet count,and C-reactive protein(CRP)compared with the non-wheezing group(P> 0.05).conclusion:1.miR-218 may participate in the pathogenesis of infantile wheezing pneumonia by targeting the negative regulation of HMGB1 expression.2.Neutrophils and eosinophils are involved in the pathogenesis of wheezing pneumonia in infants.3.Through the ROC curve analysis,miR-218 and HMGB1 could be found as a good predictor of wheeze pneumonia,which contributed to the early prediction of the occurrence of wheezing.
