| Objective To evaluate if lower extremity artery disease in middle-aged and elderly individuals with type 2 diabetes mellitus(T2DM)correlates with impedance cardiogram monitoring indexes.Methods A total of 158 T2 DM patients admitted to the general Department of Wuhan Central Hospital from December 2021 to July 2022 were retrospectively analyzed,and were divided into a control group(78 cases T2 DM group)and an experimental group(80 cases LEAD group with ankle-brachial index ≤0.9)according to the symptoms,signs and Doppler ultrasound results of both lower limbs.The experimental groups were further divided into group Ⅰ(22 cases),group Ⅱ(31 cases),group Ⅲ(18 cases),and group Ⅳ(9 cases)according to Fontaine staging.The general data,serological indexes,and hemodynamic data of the two groups were analyzed and compared.Results Age,BMI,fasting blood glucose,Hb A1 c,tri-glycerides,systemic vascular resistance(SVR),stroke systemic vascular resistance(SSVR),stroke systemic vascular resistance index(SSVRI),cardiac output(CO),and cardiac index(CI)were all statistically significant differences between the two groups(all P<0.05).Compared with the control group,SVR(r=0.350,P < 0.001),SSVR(r=0.314,P < 0.001),and SSVRI(r=0.354,P < 0.001)were positively correlated with disease,and CO(r=-0.311,P < 0.001)and CI(r=-0.294,P < 0.001)were negatively correlated with disease.Fasting blood glucose(OR=1.588,β:0.463,95%CI: 1.056-2.389),SSVR(OR=0.970,β:-0.030,95%CI:0.951~0.990),SSVRI(OR=1.059,β:0.057,95%CI:1.016~1.103),cardiac output(OR=6.670,β:1.898,95%CI:1.152~38.621)and cardiac index(OR=0.018,β:-4.036,95%CI:0.001~0.464)were independent influencing factors for lower limb artery disease in middle-aged and elderly patients with T2DM(all P < 0.05).The levels of CO(AUC: 0.701,sensitivity 71%,specificity 66%)and CI(AUC: 0.685,sensitivity 67%,specificity 65%)are of certain diagnostic value in middle-aged and elderly patients with T2 DM lower extremity arterial disease.Conclusion The levels of SVR,SSVR,and SSVRI increased and the levels of CO and CI decreased in middle-aged and elderly patients with T2 DM combined with LEAD,suggesting that impedance hemodynamic detection has a certain diagnostic value for middle-aged and elderly patients with T2 DM combined with LEAD. |
