Objective: To compare the correlation and consistency of left ventricular ejection fraction(LVEF)obtained by dual-source CT and echocardiography in patients with heart failure and non-heart failure,and to explore the value of LVEF obtained by dual-source CT in predicting heart failure,so as to provide reference for the evaluation of left ventricular function and early diagnosis of heart failure.Methods: A total of 99 patients with chronic heart failure who were hospitalized in the Department of Cardiology of the First Affiliated Hospital of Dali University from January 2019 to January 2023 were selected as the heart failure group,and 102 patients without heart failure who were hospitalized during the same period were selected as the non-heart failure group as the control group.Both groups of patients underwent dual-source CT and echocardiography during hospitalization.The left ventricular function parameters measured by dual-source CT and echocardiography were compared between the two groups.To analyzed the correlation of left ventricular ejection fraction measured by dual-source CT and echocardiography,and analyzed the predictive value of dual-source CT left ventricular ejection fraction for heart failure.Results:1.There were no significant differences in age,gender,BMI,history of hypertension,diabetes,smoking,fasting blood glucose(GLU)and low density lipoprotein cholesterol(LDL-C)between heart failure group and non-heart failure group(P> 0.05).B-type natriuretic peptide(BNP),Urea,creatinine(CREA),total cholesterol(TC),triglyceride(TG),high density lipoprotein cholesterol(HDL-C)were statistically different between the two groups(P< 0.05).2.There was significant difference between left ventricular ejection fraction(LVEF)measured by dual-source CT(DSCT)and LVEF measured by echocardiography(EC)(P< 0.001),and there was a positive correlation between them(r=0.753,P< 0.001).3.Multivariate Logistic regression analysis showed that BNP was an independent risk factor for heart failure(P< 0.001).4.In heart failure group,LVEF measured by DSCT and LVEF measured by EC were negatively correlated with BNP(r=-0.378,P< 0.001;r=-0.665,P< 0.001).5.The area under the ROC curve of BNP for predicting heart failure was 0.948,the sensitivity was84.8%,the specificity was 96.1%,and the positive cut-off value was 98 pg/ml.The area under the ROC curve of LVEF in BNP combined with DSCT for predicting heart failure was 0.953.The area under the ROC curve of LVEF of BNP combined with EC for predicting heart failure was 0.946.The value of LVEF of BNP combined with DSCT for predicting heart failure was higher than that of BNP and BNP combined with EC.Conclusion : 1.There was a positive correlation between DSCT and left ventricularejection fraction measured by echocardiography.2.When BNP was an independentpredictor of heart failure,left ventricular ejection fraction obtained by dual-source CT and echocardiography was negatively correlated with BNP in patients with heart failure.3.BNP,BNP combined with dual-source CT left ventricular ejection fraction,BNP combined with echocardiography left ventricular ejection fraction have certain predictive value for heart failure,among which BNP combined with dual-source CT left ventricular ejection fraction has the highest predictive value. |
