Objective:As a type of oncoplastic surgery,the volume-displacement technique uses gland advancement and rotation or transposition of the flap to fill the residual cavity after breast-conserving surgery.The isolated full-thickness fibrous glandular breast flap is advanced into the defect,usually achieving bilateral breast symmetry without reconstruction of the contralateral breast.For inner quadrant breast cancer with fewer glands and surgery is likely to cause deformity,the objective score and subjective satisfaction of breast cosmetic effect were used to evaluate whether the volume displacement technique has better cosmetic effect than conventional breast-conserving surgery.Methods:A total of 101 female breast cancer patients with breast-conserving surgery performed by the same operator in Huaihe Hospital of Henan University from January 2018 to January 2021 were collected.54 cases underwent volume displacement technique(observation group),and 47 cases underwent conventional breast-conserving surgery(control group).The collected clinical data included:patient age,body mass index,tumor size,breast volume,tumor to breast volume ratio,whether axillary lymph node dissection was performed,breast sagging,and breast density.The breast cosmetic effect was evaluated with the objective breast beauty rating scale and the subjective satisfaction rating scale during the postoperative re-examination.P<0.05 is statistically significantResult:The subjective satisfaction rate of the observation group was 79.6%,and the subjective satisfaction rate of the control group was 51.1%.Compared with the two groups,the observation group was better than the control group,and the difference was statistically significant(X~2=11.250 P=0.024).The objective score excellent and good rate of the observation group was 74.1%,and the objective score excellent and good rate of the control group was 42.5%.Comparing the two groups,the breast cosmetic effect of the observation group was better than that of the control group,and the difference was statistically significant(X~2=10.924 P=0.004).The age,body mass index,tumor size,breast volume,tumor-to-breast volume ratio,axillary lymph node dissection,breast sagging,and breast density had no significant effect on breast aesthetics(P>0.05).Conclusion:For breast cancer in the inner quadrant,both subjective satisfaction and objective breast aesthetic scores,the excellent and good rate of volume displacement technique was higher than that of conventional breast-conserving surgery.In terms of breast symmetry,breast concavity,nipple displacement distance,breast compliance,etc.,volume displacement technology is better.There were no differences in skin color and postoperative scarring.In terms of texture and elasticity,it is not yet certain that the volume displacement technique is superior to conventional breast conserving surgery,and long-term follow-up is required to draw conclusions.Through the statistical analysis of the difference between the observation group and the control group,it can be considered that for breast cancer in the inner quadrant,the volume displacement technique can better preserve the beauty of the affected side breast than conventional breast conserving surgery.It has high application value in clinical practice,and it is recommended that clinicians learn and promote its use.
