ObjectiveThis study was a correlation analysis between hepcidin level and nutritional status in patients with chronic heart failure,in order to provide an early and effective diagnosis and treatment index for nutritional status assessment and nutritional support therapy in patients with chronic heart failure.MethodsIn this study,75 patients with chronic heart failure admitted to the First Affiliated Hospital of Dali University from April 2022 to October 2022 were selected as the experimental group, and 50 patients with normal cardiac function were selected as the control group. The hepcidin level of all selected patients was measured and the collection and examination of general clinical data such as blood routine,CRP,liver and kidney function, cardiac ultrasound and nutritional status were improved.Organize data and build an Excel database.Using SPSS(Statistical Product and Service Solutions)25.0 and Graphpad prism 8.0 software for statistical analysis and graphing, respectively. To analyze the correlation of hepcidin levels with nutritional status in patients with chronic heart failure.Results1.Comparison of general clinical data: BMI,smoking history, drinking history, heart rate and diastolic blood pressure between patients with heart failure and those with normal cardiac group were not statistically significant(P>0.05);However,the proportion of male patients in the experimental group was higher than that in the control group, the average age of the experimental group was greater than that of the control group, and the systolic blood pressure of the patients in the experimental group was lower than that in the control group, and the difference was statistically significant(P<0.05).2.Blood cell analysis: patients with heart failure had lower platelet and lymphocyte counts than those in the normal cardiac group, and the ratio of neutrophils to lymphocytes was higher than that in the normal cardiac group, and the difference was statistically significant.Compared with the control group, there were no significant differences in the absolute values of white blood cells, neutrophils, red blood cells, hemoglobin and CRP.3.Comparison of blood biochemical indexes: the levels of TBI,DBI,IBI,AST,CREA,UA,and Urea in patients with heart failure were higher than those in the group with normal cardiac function; The levels of ALB,GFR,TC,and TG in patients with heart failure were lower than those in the group with normal cardiac function, and the difference was statistically significant.4.The level of hepcidin in patients with heart failure was significantly higher than that in the control group, but the level of hepcidin in patients with heart failure was not correlated with RBC,HB,BNP,LVEF,left ventricular end-diastolic diameter and left atrial inner diameter, and the difference was not statistically significant.5.The GNRI scores of heart failure patients were significantly lower than those in the control group, while the CONUT score was significantly higher than that in the control group, and the proportion of patients with CONUT detecting malnutrition risk was significantly higher than that of GNRI,which may be more valuable for the diagnosis of malnutrition in patients with CHF.6.There was no correlation between nutrition-related scores and RBC and HGB in patients with heart failure, and the difference was not statistically significant.7.The results of binary logistics regression analysis showed high levels of hepcidin and high level CONUT scores were independent risk factors for CHF.8.Based on the GNRI,the nutritional status was divided into the group without malnutrition risk and the group at risk of malnutrition, and there was no significant difference in hepcidin between the two groups.Based on the CONUT,the nutritional status was divided into no malnutrition group, mild malnutrition group and moderate to severe malnutrition group, and there were no significant differences between and within the three groups, and the hepcidin was analyzed in the two nutritional assessment indexes, and the results showed that there was no correlation between hepcidin and nutritional status.Conclusion1.In patients with chronic heart failure, the level of hepcidin is increased, and the nutritional status is significantly reduced.2.Hepcidin levels in patients with chronic heart failure are not correlated with nutritional status.
