| ObjectiveExploring the classification and influencing elements of fear of childbirth in pregnant women, and correlating each subtype with perinatal depression,is the aim.In order to provide medical staff with research ideas and theoretical basis,to provide appropriate psychological care and formulate effective intervention strategies for pregnant women with fear of childbirth and perinatal depression.Method1.From the Department of Obstetrics at a tertiary class hospital in Yunnan Province,285 expectant mothers who had regular prenatal examinations, chosen via convenience sampling from March to October 2022 to serve as the research objects. After being approved by the hospital ethics committee,Childbirth attitude questionnaire(CAQ),Edinburgh postnatal depression scale(EPDS),and Childbirth Attitude Questionnaire(CAQ)were used to investigate childbirth.Pregnant women’s fear of childbirth was analyzed by childbirth attitude questionnaire, and the count data were expressed by frequency and percentage.Normal distribution-conforming measurement data was expressed by mean and standard deviation, while those that did not conform were represented by the median-quartile interval. To assess the influencing factors of fear of childbirth, the tidy LPA package based on R4.1.0 software was utilized for potential profile analysis, and SPSS23.0 was utilized for disordered multi-classification logistic regression analysis to evaluate the influencing factors of fear of childbirth.P<0.05 indicated that the difference was statistically significant.2.Chi-square test was used to explore the relationship between fear classification and the incidence of depression. The rank sum test of K independent samples was used to explore the relationship between the classification of fear of childbirth and perinatal depression score.spearman correlation analysis was used to explore the pertinence between fear score and perinatal depression score. All the above were considered to have statistical significance with P<0.05.Results1.An analysis of the potential profile of fear of childbirth among pregnant women was conducted, with the results indicating that the most suitable group of pregnant women was divided into three categories.47.4% of the group had minimal fear of childbirth, referred to as the "low fear of childbirth group",while 39.3% of the group had a moderate level of fear of childbirth, known as the "middle fear of childbirth group".13.3% of pregnant women had the highest degree of fear of childbirth, which was called "high fear of childbirth group".Among them, the proportion of the "moderate fear of childbirth group" and "high fear of childbirth group" is more than half, indicating that the incidence of fear of childbirth is at a high level. The disordered multinomial Logistic regression analysis showed that the number of previous deliveries, whether the delivery was prepared or planned, and the problems of childbirth to worry about were the predictors of the potential categories of childbirth fear, and the difference was significant(P < 0.05).2.A significant distinction between the occurrence of perinatal depression and the categorization of fear of childbirth was observed when the perinatal depression threshold was 5 or 9.5(P<0.05).The scores of parturition fear and perinatal depression were significantly different(P<0.05).The scores of fear of childbirth and perinatal depression were significantly different(P<0.05).Conclusion1.The optimal potential profile of fear of childbirth was divided into three groups, which were named as "low fear of childbirth group","medium fear of childbirth group" and "high fear of childbirth group".The occurrence of fear of childbirth in pregnant women is influenced by the previous delivery, whether the delivery is adequately prepared or planned, and the problems of the delivery.2.The higher the degree of fear of childbirth, the higher the incidence of perinatal depression;Parturient fear classification was related to perinatal depression score. After pairwise comparison, different fear classification had different perinatal depression score. There was a positive correlation between the fear of childbirth score and the perinatal depression score. The higher the fear of childbirth score, the higher the perinatal depression score. It can lay a foundation for medical staff to carry out early screening of negative emotions of pregnant women, propose intervention measures, and improve pregnancy and delivery experience. |
