| Objectives:This article focuses on the current poor quality of pharmaceutical consultations in hospitals,and explores the establishment of a pharmaceutical consultation quality control system using the PDCA cycle management method.The focus is on carrying out quality control circle activities to standardize pharmaceutical consultations,focusing on improving the efficiency of consultations,in order to improve the quality of pharmaceutical consultations in a certain hospital.Methods:1.Analyze the effect of pharmaceutical consultation in the early stage of a hospital,and find out the factors affecting the effect of consultation through single factor and multiple factor analysis.2.The quality control evaluation system of pharmaceutical consultation was initially established: firstly,the factors affecting the quality of pharmaceutical consultation were searched by brainstorming and classified and summarized by fishbone diagram analysis;Secondly,the true cause is listed as the evaluation index by the true cause inspection method,and Cronbach is used α The coefficient is used to test the reliability of the determined index,and then the entropy weight method is used to calculate the weight of the evaluation index;Finally,with the help of the SPO model,the consultation quality control evaluation system is formed to determine the quality control indicators.3.Operation of the pharmaceutical consultation quality control system: establish a QCC team to carry out quality control activities of the quality control circle around the theme of improving the efficiency of the consultation,analyze the factors that lead to ineffective pharmaceutical consultation intervention,determine the true cause,formulate and implement countermeasures based on the actual situation of the department,and evaluate the effectiveness of the quality control circle and standardize it according to the quality control indicators.Results:1.The overall adoption rate of pharmaceutical consultation opinions in a hospital from July to December 2021 was 97.88%,and the effective rate of treatment was 86.99% after the adoption of the opinions.The adoption of pharmacist consultation opinions by doctors would help improve the effect of consultation and intervention(OR=10.754,95% CI=1.842~62.776).2.The quality control system of pharmaceutical consultation was initially established: 3 first-level indicators,5 second-level indicators and 9 third-level indicators,with the maximum weight of 0.132 and the minimum weight of 0.024.It is clear that the quality control indicators of pharmaceutical consultation mainly include the number of consultation,the effective rate of consultation,the timely rate of consultation,the adoption rate of consultation,and the qualification of consultation personnel.3.After the quality control activities in the quality control circle,the effective rate of pharmaceutical consultation and intervention increased from 82% to 93%,the target achievement rate was 104.67%,and the progress rate was 13.41%.All circle members have been significantly improved in six aspects:communication and coordination ability,problem-solving ability,self-confidence,enthusiasm,team cohesion,and quality control skills.Standardize the following successful results:(1)consultation opinion template;(2)Standardized consultation process;(3)Communication and coordination system between Pharmaceutical Department and clinical departments.Conclusions:This study applies PDCA cycle to pharmaceutical consultation services,clarifies the quality control indicators of pharmaceutical consultation,uses the quality control circle approach to carry out pharmaceutical consultation quality control activities,improves the quality of pharmaceutical consultation,provides practical experience for realizing standardized and value-based pharmaceutical consultation mode,and also provides reference and more ideas for pharmaceutical consultation in grass-roots hospitals.However,there are still deficiencies in the whole PDCA cycle that need to be reviewed to provide a breakthrough for the next PDCA cycle. |
