| Objective: The purpose of this study was to investigate the effectiveness of liver thinning and spleen manipulation in the intervention of irritable bowel syndrome with diarrhea(hepatodepressed and spleen deficiency).And make the liver thinning and spleen massage form a prescription for easy promotion.Methods: In this study,60 patients with TCM syndrome diagnosed in accordance with the diagnosis of diarrhea-type irritable bowel syndrome by Western medicine were collected from the outpatient patients of Tuina Department and Gastroenterology Department of the Affiliated Hospital of Changchun University of Chinese Medicine,and 60 patients were diagnosed according to the differentiation of TCM,which were classified into 30 cases in the liver thinning and spleen massage treatment group and oral drug control group.The treatment group was treated with liver thinning and spleen massage,and the control group was treated with oral pain-laxative granules.The treatment group had 27 minutes of each massage,5times a week,and 4 weeks for 1 course of treatment;The control group took the drug for 4weeks and 1 course of treatment.After one course of treatment,the overall response rate of the two groups of patients was observed,and the scores of liver depression and spleen deficiency syndrome,the score of the Irritable Bowel Syndrome Quality of Life Scale(IBSQOL Scale),the score of the Anxiety and Depression Measure Scale(HAD Scale)and other indicators before and after treatment were observed,and the clinical efficacy of the liver thinning and spleen massage method in the treatment of diarrhea-type irritable bowel syndrome was evaluated.Results: The patients included in this project were all from the Department of Tuina and the Department of Hepatology,Spleen and Gastroenterology of Jilin Provincial Hospital of Traditional Chinese Medicine,and it was estimated that 60 patients with carpal tunnel syndrome and diarrheal irritable bowel syndrome were collected,60 cases were actually included,30 cases were treated in the treatment group,and 0 cases were shed.There were 30 cases in the control group,and 0 cases were shed.A total of 30 patients were finally completed in this study,and 30 patients were included in each group.The comparison of the efficacy of each is as follows:(1)In terms of clinical comprehensive efficacy: the number of clinical healings in the treatment group was 5 cases,the number of obvious effects was 13 cases,the effective number was 8 cases,and the total effective rate was 86.67%;The number of clinical recoveries in the control group was 2 cases,the number of apparent effects was 7 cases,and the effective number was 14 cases,with a total effective rate of 76.67%.The comparative efficacy of the two groups was statistically significant(P<0.05),and the efficacy of the treatment group was better than that of the control group,suggesting that the clinical symptoms of IBS-D patients with liver depression and spleen deficiency were better than those of the oral drug group.(2)In terms of the total score of TCM symptoms,the symptoms of both groups were significantly relieved after 4 weeks of treatment compared with before treatment,and the treatment group was better than that of the control group(P<0.05);(3)In terms of single symptom points in traditional Chinese medicine,after 4 weeks of treatment,the symptoms of patients in both groups were improved to a certain extent,and the difference between the control group and the treatment group was small in the relief of symptoms(P>0.05),and the treatment group was better than the control group(P<0.05)in the five items of abdominal pain and bloating,diarrhea frequency,irritability,chest and hypochondrium,and sluggishness;(4)In terms of the improvement of the score of the irritable bowel syndrome quality of life scale(IBS-QOL scale),the scores of the IBS-QOL scale in both groups were significantly improved in some dimensions compared with the previous one,and the increase in points in the treatment group after completion of treatment was higher than that in the control group(P<0.05);(5)Anxiety and depression measurement scale(HAD scale)scoring index of the two groups after treatment: The before and after comparison between the two groups of anxiety and depression HAD score group had statistical differences in both HADa and HADd(P<0.05),and the scores were lower than those before treatment,indicating that the treatment methods of both groups had an improvement effect in these four aspects.The comparison between the two groups after treatment was statistically different in both HADa and HADd(P<0.05),indicating that there were obvious differences in the improvement of these two groups,and the treatment group was better than the control group.Conclusion:(1)The effect of liver thinning and spleen massage method in improving the score of traditional Chinese medicine syndrome,anxiety and depression HAD score in IBS-D patients was more prominent.(2)The liver thinning and spleen massage method is more effective in improving the four dimensions of anxiety,behavioral disorders,health worries,and eating and avoidance in IBSQOL scores of IBS-D patients,and the effect is comparable in the four dimensions of physical awareness,social function,sexual behavior,and interpersonal relationships,but the liver thinning and spleen massage method has the advantages of green and less health side effects,and can become a new method of alternative drugs to a certain extent.(3)Compared with conventional oral paintherapy granules,the treatment group had advantages in many aspects,especially obvious improvement in emotional sentiment,and no adverse reactions,good safety,and worthy of further promotion. |
