Distribution Of Syndrome Elements And Correlation With Lipid Metabolism In Overweight/obese Patients With Type 2 Prediabetes

Objective: To analyze the syndrome differentiation of TCM syndrome elements in overweight/obese patients with type 2 prediabetes,and to analyze the characteristics and regularity of the disease in patients with type 2 prediabetes through the study of the correlation between syndrome elements and lipid metabolism,and to further explore the differences of lipid metabolism of each syndrome element,in order to provide the basis for TCM syndrome differentiation and treatment of type 2 prediabetes and provide a reference for TCM prevention and treatment.Methods: In this study,257 overweight/obese patients with type 2 prediabetes screened in the endocrine clinic and community of the Affiliated Hospital of Changchun University of Traditional Chinese Medicine were collected,including basic information,such as gender,age,disease duration,family history of diabetes,weight,height,waist circumference,etc.,as well as clinical information,such as four diagnostic information,fasting blood glucose,2h blood glucose after glucose load,fasting insulin,glycosylated hemoglobin,triglycerides,total cholesterol,high-density lipoprotein cholesterol,low-density lipoprotein cholesterol and other biochemical indicators.Statistical analysis of TCM syndrome element distribution and correlation between syndrome elements and lipid metabolism.Results: 257 patients were finally included in this study.There were 94 males(36.58%)and 163 females(63.42%),and the age distribution of the patients was mainly between 30 and60 years.Ninety-four patients(36.58%)had a family history of diabetes and 163 patients(63.42%)had no family history of diabetes.According to statistics,there are 13 syndrome elements of disease location,most of which are spleen.There were 11 solid syndrome elements,most of which were wet.There are 6 deficiency syndrome elements,most of which are qi deficiency.Cluster analysis of the syndrome element data with a constituent ratio greater than 5% showed that when the scale was 15,it could be divided into five major categories.The first type: dampness,spleen,phlegm;The second type:qi stagnation,liver,blood stasis;The third type: qi deficiency,yang deficiency;The fourth type: kidney,heart,yin deficiency;The fifth type: heat,stomach.According to the main syndrome elements with the highest proportion,BMI,DBP and LDL-C in the spleen group were higher than those in the non-spleen group(P < 0.05),BMI,WC,DBP and LDL-C in the wet group were higher than those in the non-wet group(P < 0.05),and FINS in the qi deficiency group was lower than that in the non-qi deficiency group(P < 0.05).Conclusion:1 This study suggests that the distribution of overweight/obese type 2 prediabetes is mainly concentrated in young and middle-aged people,and some people have a family history of diabetes.Therefore,at the same time of clinical treatment,it is advocated to start the screening of diabetes in such populations as early as possible and give intervention treatment.2 After the syndrome elements of the four diagnostic information of the included patients were differentiated,the data were statistically processed,and the results showed that the syndrome elements of disease location were most concentrated in the spleen,the solid syndrome elements were most concentrated in the dampness,and the virtual syndrome elements were most concentrated in the qi deficiency.The results of cluster analysis showed that it could be divided into five categories: The first type: dampness,spleen,phlegm;The second type:qi stagnation,liver,blood stasis;The third type: qi deficiency,yang deficiency;The fourth type: kidney,heart,yin deficiency;The fifth type: heat,stomach.3 There is a correlation between the main syndrome elements and some physicochemical indicators.Overweight/obesity in these patients is closely related to spleen and dampness.Patients in spleen group and dampness group are easy to produce hypertension on the basis of diabetes,and LDL-C elevation is the most common dyslipidemia.Overweight/obese patients with type 2 prediabetes have overall higher TG/HDL-C levels and are more likely to develop type 2 diabetes.
