Exploring Professor Yangnan’s Experience In Treating Chronic Glomerulonephritis Based On Data Mining Techniques

Objective: The purpose of this study is to retrospectively analyze the outpatient cases of chronic glomerulonephritis treated by Professor Yangnan,to deeply explore the potential rules of his prescriptions and medicines,to summarize his experience of using the six meridians to treat this disease,and then to explore his clinical treatment thinking,in order to promote the popularization of Chinese medicine clinical experience and academic ideas,and to provide more reference basis for the Chinese medicine treatment of this disease.Methods:In this study,outpatients diagnosed with primary chronic glomerulonephritis by Professor Yang Nan of the Department of Nephrology,First Clinical Hospital of Jilin Academy of Traditional Chinese Medicine during December 2018-December 2021 were studied,and their medical records were systematically analyzed and combined with Professor Yang Nan’s herbal prescriptions to explore their treatment ideas and treatment methods in depth.Results: A total of 200 patients with chronic glomerulonephritis met the study criteria and 200 prescriptions were obtained in this study,and after systematic analysis,we concluded the following.1.In this study,the patients were predominantly male,accounting for 70%.The main age of the disease was concentrated between 40-70 years old,accounting for 91% of the total number of patients in this study.2.A total of 67 clinical symptoms(excluding tongue and pulse)were involved in this study,and the top clinical symptoms(excluding tongue and pulse)were weakness,dry mouth,poor sleep,back pain,frequent nocturnal urination,distress,shortness of breath,bitter mouth,dry throat,poor appetite,thirst,loose stools,and fear of cold.From the clinical symptoms,the common six meridians of chronic glomerulonephritis treated by Professor Yang are: Shao-Yin,Tai-Yin,Shao-Yang,and Sun.3.The tongue of chronic glomerulonephritis treated by Prof.Yangnan is mainly red with yellow coating,and patients with tooth-marked tongue and yellow greasy coating can also be seen clinically,which belong to Shaoyang,Taiyin,and Shaoyin heat transformation;the pulse is mainly deficient,with 13.8% and 11.9% of the overall pulse being stringy and thin,respectively,and the pulse can also be sunken and weak,slippery and thin,which belong to Shaoyang,Shaoyin,and Taiyin.4.Professor Yang used 142 drugs for the treatment of chronic glomerulonephritis,and the top 10 most frequently used sutra prescriptions from the Astragalus,Poria,and Chai Hu categories were Astragalus(173),Sangchuang(124),Poria(114),Oyster(101),Dragon Bone(96),Chai Hu(94),Cuscuta(88),Atractylodes(84),Scutellaria(83),and Radix et Rhizoma(82).5.According to the statistical data,among the four qi and five tastes,the highest percentage of cold drugs was used,reaching 40.3%,followed by warm drugs with 32.3%and flat drugs with 21%.Among the five tastes,sweet drugs were used most frequently,reaching 36.9%,followed by bitter drugs,accounting for 22.7%,and pungent drugs,accounting for 20.1%;the meridians were dominated by drugs entering the spleen,kidney,liver,lung,stomach and heart meridians,accounting for 83.6% of the total amount of drugs.6.In total,Professor Yang used 90 groups of commonly used combinations of Chinese medicines for the treatment of chronic glomerulonephritis,including 53 groups of commonly used 2-medicine combinations,31 groups of 3-medicine combinations,and 5groups of 4-medicine combinations,involving a total of 20 Chinese medicines.7.A total of 26 core drug combinations and 13 potential new prescriptions were obtained from Prof.Yangnan,and 5 were retained in combination with Prof.Yang’s clinical experience,namely:(1)Che Qian Zi-Roasted Licorice-Slippery Rock-Ze Xie Di,(2)Xiao Fennel-Radix Pseudostellariae-Gun Ginger-Dry Ginger,(3)Sheng Di Huang-Huang Lian-Dragon Bone-Lily-Ge Gen,(4)Wine Rhubarb-Poria-Atractylodes-Yi Mao Cao,(5)Wu Bei Zi-Xiao Thistle-Lotus Root-Huang Bai.Conclusion: 1.Prof.Yangnan pointed out that the main evidence of chronic glomerulonephritis is caused by the deficiency of the righteousness,while the standard evidence is caused by the evil offending the six meridians.The treatment should be based on the principle of "supporting the righteousness and eliminating the evil",and the treatments of "consolidating deficiency","dispelling wind","clearing heat",and "relieving dampness" can be used flexibly."Dampness" and other treatment methods can be flexibly added to achieve the best results.The disease is located in the kidney and is closely related to the liver,spleen and lung.2.Professor Yang’s treatment of chronic glomerulonephritis is based on the six meridians: Shao-Yin,Tai-Yin,Shao-Yang and their combination.3.In the treatment of chronic glomerulonephritis,Prof.Yangnan takes the six meridians as the basis,based on the addition and reduction of Huang Qi,Fu Ling,Chai Hu and other types of meridian formulae,which are good at tonifying the internal organs,flexible in the use of drugs,and emphasize prevention and regulation.
