Objective: To explore the effect and safety of total transvaginal hysterectomy combined with sacrospinous ligamentamisectomy for pelvic organ prolapse.Methods: Screening of patients with pelvic organ prolapse in the obstetrics and gynecology department of our hospital from January 2019 to June 2022,A total of 86 people underwent surgical procedures,Divided into two groups according to the treatment modality performed,A total of 54 individuals were screened in the observation group,The surgical method was transvaginal total vaginal hysterectomy + anterior and posterior vaginal wall repair combined with sacroacanthous ligamententation;A total of 32 individuals were screened in the control group,The technique is total vaginal hysterectomy + anterior and posterior vaginal wall repair;Study analyzed the preoperative,intraoperative and postoperative related indicators in the two groups,General clinical data of the preoperative patients,Including age,BMI,the number of deliveries,and the delivery history of the giant baby;Intraoperative indicators: operation time,intraoperative bleeding amount,hospitalization time and hospitalization cost;Postoperative symptom improvement,complications(including bleeding,infection,urinary incontinence,defecation disorders,etc.),and patient POP-Q stage,prolapse recurrence,and quality of life score in the first month and sixth month.Results: There was no significant difference in age,BMI,deliveries,and macrosomal deliveries between the two groups(P> 0.05);the mean operation time in the observation group was(93 ± 6.10)min was significantly longer than the control group(76 ± 1.26)min,and the difference was significant(P <0.05).There was no significant difference in intraoperative bleeding,urinary retention,hospital stay,or hospital cost between the two groups(P> 0.05);postoperative prolapse recurrence rate was lower(1.85%)than the control group(9.37%)(P<0.05),while postoperative symptom improvement,complications(urinary incontinence,rectal dysfunction),PFDI-20,and no significant difference between the two groups(P> 0.05).Conclusion: This study found that both surgical methods can significantly reduce the symptoms of pelvic organ prolapse and improve the quality of life of patients,the observation group than the control group relatively long,but the postoperative prolapse recurrence rate is significantly reduced,and did not increase the risk of bleeding,infection,urinary incontinence and defecation complications,therefore,it can be said that total vaginal hysterectomy +vaginal anterior and posterior wall repair combined sacral ligament fixation is a feasible,effective and safe treatment of pelvic organ prolapse surgery.
