Objective : With the rapid development of implant technology and the update of surgical materials,many scholars at home and abroad have proposed methods to reconstruct the alveolar ridge contour of horizontal bone defects,so that the implant can obtain sufficient initial stability.This study retrospectively analyzed the implantation of35 cases with a total of 50 implants to explore the effect of applying digital technology to design preformed titanium mesh in the implant restoration of horizontal alveolar bone defects in the anterior region and the effect of conventional GBR surgery.The difference in effect aims to provide clinicians with a safe,convenient and highly operable surgical method,and provide an intuitive and effective implant solution for more patients with dentition defects.Methods : The experimental group selected 18 patients with medical records who underwent digital preformed titanium mesh bone grafting in the Department of Stomatology,Fuyang People’s Hospital,Anhui Province from January 2021 to December 2022,including 12 males and 6 females,aged 18-65 years,with an average age of 39.61±15.92 age.The control group selected 17 cases of conventional GBR bone grafting in the Department of Stomatology,Fuyang People’s Hospital,Anhui Province from January 2021 to December 2022,including 10 males and 7 females,aged18-62 years,with an average age of 38.41±12.70 years.All patients underwent CBCT photography and 3Shape oral scans.In the 3shape Implant Studio software,the implant position was placed and the implant guide was generated according to the restorationoriented design.During the operation,the implant guide was used for accurate implantation.The oral scan data and implant position design data of the experimental group were handed over to the orthopedic 3D laboratory of our hospital to pre-restore the alveolar bone contour and print the plastic jaw model according to the implant position and implant principles.The finished titanium mesh is pre-bent,and the preformed titanium mesh is used for precise bone grafting during the operation.The control group used conventional periosteal bone powder bone grafting to restore the ideal jaw contour according to the surgeon’s experience.Collect and record relevant data such as patient pain score,overall patient satisfaction,postoperative complication rate,preoperative CT,postoperative CBCT,six-month postoperative CBCT,and postoperative red aesthetic score(PES),using SPSS The software was used for statistical analysis of the collected data,and an independent sample t-test was used to analyze the two groups of data to compare the difference in the bone formation effect of the preformed titanium mesh bone graft and the conventional GBR bone graft in the horizontal bone defect of the anterior tooth region.Results : 1.The pain score(1.94±0.97)and overall satisfaction(7.65±0.79)of the experimental group were not significantly different from those of the control group(1.71 ± 0.77)and overall satisfaction(7.47 ± 0.87),p> 0.05.2.In the experimental group,1 case was exposed to titanium mesh(cover cap exposure)and 2 cases had paresthesia within 4 weeks after operation,and 1 case had wound infection,1 case was exposed to titanium mesh,and 2 cases still had paresthesia within 4-6 weeks;Within 4weeks after operation,there were 2 cases of periosteum exposure and 1 case of paresthesia,and only 1 case of paresthesia within 4-6 weeks after operation.3.The alveolar crest bone width increment(3.41 ± 1.18)of the experimental group was significantly different from that of the control group(2.50 ±0.43)at 6 months after operation,and the alveolar bone width increment of 5mm below the alveolar crest of the experimental group was 6 months after operation There was no significant difference(2.75±0.70)compared with the control group(2.67±0.78).After 6 months,the labial alveolar bone thickness of the experimental group(2.77 ± 0.61)was significantly different from that of the control group(1.99 ± 0.55).It can be considered that the preformed titanium mesh is more effective than the conventional GBR in the repair of bone defects in the anterior region,especially in the restoration of the top of the alveolar ridge and the labial bone of the implant neck.4.After crown restoration,the red aesthetic score of the experimental group(10.53±1.33)was slightly higher than that of the control group(9.12±1.73),p<0.01,and the difference between the two groups was statistically significant.Conclusion : In this study,the use of preformed titanium mesh to restore the bone defect in the anterior tooth area and simultaneous implant implantation can obtain an ideal increment of alveolar bone width and ideal reconstruction thickness in the labial bone wall.The preformed titanium mesh group has a good aesthetic effect in the initial repair,and the patients with low incidence of postoperative complications have a good perception,and the long-term effect of osteogenesis needs to be further observed and evaluated.
