Comparison Of The Curative Effect Of Rehabilitation Exercise Under CPM And Guidance Of Rehabilitation Physician After Primary Artificial Knee Arthroplasty

Objective(s): This study compared the efficacy and economic indicators of continuous passive motion(CPM)versus rehabilitation physician-directed rehabilitation exercises after initial prosthetic knee arthroplasty(TKA)to investigate more effective rehabilitation exercises in the early postoperative period after prosthetic knee arthroplasty.Methods: Collected 75 cases of patients who underwent initial artificial total knee arthroplasty in the Department of Orthopedics of First People’s Hospital of Yunnan Province,between 01/2022 and 08/2022,all patients were grouped randomly into 3groups as follows:(1)rehabilitation physician group,the patient-directed rehabilitation under the guidance of rehabilitation physicians;(2)CPM training group;(3)comprehensive rehabilitation therapy group(CPM combined with rehabilitation physicians).Visual Analogue Scale(VAS),Knee Society Score(KSS),The Western Ontario and Mc Master Universities(WOMAC)score,Range of motion(ROM),Knee circumference,postoperative complications and rehabilitation costs of the patients in the three groups were compared at 3 days,1 week,2 weeks,6 weeks,3 months and 6months after surgery respectively.Results: Through comparing the three different rehabilitation modalities,after the initial total knee arthroplasty:(1)The patients’ knee function improved,the difference has statistically significant(P<0.05).(2)Lowest pain score showed at 3 days and 6 weeks postoperative in the comprehensive rehabilitation group,the difference has statistically significant(P<0.05).(3)Greatest knee flexion angle showed at 1 week postoperative in the CPM group(P<0.05).(4)KSS scores of patients at 2 weeks postoperative in the comprehensive rehabilitation group were better than that of the CPM group(P<0.05).(5)WOMAC scores of patients at 3 days postoperative in the rehabilitation physician group were better than those in the CPM group(P<0.05).(6)Degree of improvement of knee swelling among the three rehabilitation training modalities had no difference(P>0.05).(7)The incidence of postoperative complications among the three groups had no difference(P>0.05).(8)Using CPM for rehabilitation training resulted in the lowest rehabilitation costs for patients.Conclusion(s): All three rehabilitation training methods can improve the knee joint function of patients,with CPM showing an advantage in improving the knee flexion angle in the early postoperative period,while active functional exercise guided by rehabilitation physicians is more beneficial for the functional and coordination recovery of the knee joint.From an economic point of view,adopting more effective and economical rehabilitation treatment options can reduce the medical burden of patients and also greatly improve the efficiency of medical resources,thus saving medical costs for society,which has important social and economic significance.
