The aging in China is deepening,survival time of the elderly is lengthening,and the proportion of the elderly with chronic diseases is increasing.According to data released by the National Health Commission,by the end of 2018,there were more than 180 million elderly people with chronic diseases in China,and the number of elderly people with chronic diseases in rural areas had continued to increase.Due to the weakening of rural family pension function,the imperfect social pension system and the low medical level,the medical and pension service needs of rural elderly with chronic diseases are more difficult to be effectively met,which seriously affects their quality of life and needs to be solved urgently.Taking the elderly with chronic diseases in rural areas as the research object,this paper explores the demand and supply system construction of integration of medical and nursing care service for the elderly with chronic diseases in rural areas by combining quantitative and qualitative methods.The research content is mainly divided into the following parts.Firstly,the theory of holistic governance and the theory of healthy aging are combed to lay a theoretical foundation for subsequent empirical research and analysis.Secondly,taking X county of A province as an example,through the survey data to study the integration of medical and nursing care service demand of rural chronic disease elderly,using the ordered logistics model to analyze its influencing factors.Thirdly,through the content analysis of the policy text and the investigation of the implementation of the policy,this paper discusses the supply of integration of medical and nursing care service for the elderly with chronic diseases in rural areas,and through the qualitative analysis of interview data,this paper studies the achievements and problems of the supply of integration medical and nursing service for the elderly with chronic diseases in rural areas.Finally,it puts forward policy suggestions on the optimization of the service supply system for the rural elderly with chronic diseasesIt is found that the policy of integration of medical and nursing care as an important measure for the construction of new pension service system has brought vitality to the development of rural pension services,improved the health literacy of rural chronic elderly,enhanced the ability of health management of the elderly,and improved the satisfaction of the elderly to rural pension services.Under the multi-sectoral collaborative governance,the management of chronic diseases in rural areas has achieved initial success,but there are still some problems in the development of rural medical-nursing combined service,which need to be solved urgently.For example,the quality of medical service is low,and it is difficult to meet the high-quality medical needs of the elderly with chronic diseases.Pay more attention to medical care than maintenance,the elderly care needs are not given enough attention,the effective supply of maintenance services is seriously insufficient.Severe shortage of talent,insufficient funds and lack of strong managers lead to the lack of impetus for the long-term development of services.Long-term care security mechanism is not perfect,disabled,semidisabled elderly long-term care needs are difficult to meet.Therefore,in the next step,it is necessary for the government to improve the ability of collaborative governance,improve the supply system of medical and nursing service for rural elderly with chronic diseases from the aspects of service foundation,service mode,information platform and guarantee mechanism,and try to meet the diversified needs of the elderly with chronic diseases and promote the healthy aging of the elderly in rural areas.
