Objective:In this study,data mining technology and TCM health status identification system were applied to explore the distribution characteristics of syndrome elements and medication rules in the clinical diagnosis and treatment of constipation by Professor Li Candong,in order to summarize his clinical experience and academic thoughts in the diagnosis and treatment of constipation,and provide reference for the clinical diagnosis and treatment.Methods:A total of 355 medical records were collected from 153 constipation patients who met the inclusion criteria in Professor Li Candong’s outpatient department from September 2017 to December 2022.The basic data,four diagnosis information and prescription drugs of the collected medical records were input into Excel to form a medical data table for preprocessing and standardization,and syndrome element analysis was carried out through the health status system of traditional Chinese medicine.Frequency statistics,association rule analysis and cluster analysis were performed on basic information,symptoms,syndrome elements,drug classification,sex and taste normalization of patients.The clinical experience and academic thought of Professor Li Candong in the treatment of constipation were summarized based on the medication rules of Professor Li Candong.Results:1.General status: In this study,there were 153 patients with constipation,of which 100 were female,with a frequency of 65.4%,and 53 were male,with a frequency of 34.6%.Most of the patients were aged from 31 to 80 years old,and middle-aged and elderly(86.93%).2.Syndrome element statistics: Frequency analysis of 355 diagnoses showed that the top three syndromes of total disease locations were large intestine(76.5%),spleen(49.0%)and liver(41.8%).The top three solid elements were qi stagnation(79.7%),heat(59.5%)and dampness(53.6%).The top three syndroms of total deficiency were Yin deficiency(82.4%),Qi deficiency(73.9%)and blood deficiency(68.6%).3.Syndrome type statistics: Association rule analysis and cluster analysis were carried out on syndrome elements of disease location and disease frequency ≥10%.Constipation in Professor Li Candong’s clinic was mainly characterized by syndrome of liver qi stagnation,syndrome of blood deficiency and intestinal dryness,syndrome of qi deficiency of spleen and stomach,and syndrome of liver and kidney Yin deficiency.4.Medication rules: In Professor Li Candong’s prescription for the treatment of constipation,licorice,which was used in harmony with other medicines,was used for a total of 181 kinds of traditional Chinese medicine,with a frequency of 3,483 times.There were 26 kinds of drugs with a frequency greater than or equal to 10%,with a total frequency of72.61%.The top 12 high frequency drugs were: Fructus aurantii,pinellia pinellia,Tangerine peel,bupleurum bupleurum,poria cocos,Fructus paeoniae,raw rhubarb,Astragalus,Shenqu,Scutellaria scutellaria and salvia miltiorrhiza.Drug efficacy: Qi-regulating drug used the highest frequency(12.29%);Followed by tonifying deficiency medicine(12.00%);The next was phlegm-reducing,cough relieving and asthmatic drugs(9.84%),and the last was purging drugs(9.19%).These four drugs were the core drug efficacy categories of Professor Li Candong’s treatment of constipation.Drug taste: warm drugs(26.27%)were the most;The second was the mild drugs(24.81%),the mild cold drugs(23.05%),and the warm,mild and mild cold drugs accounted for the vast majority.The frequency of use of sweet drugs was the highest(44.78%),followed by spicy drugs(39.76%).Again,bitter drugs(38.46%).Drug normalization: Drug normalization with frequency ≥10% were spleen meridian,lung meridian,stomach meridian,heart meridian,liver meridian and large intestine.5.The main drug pairs for the treatment of constipation are bamboo Shauru pinellia,Tangerine peel-bamboo Shauru,magnolia officinalis-Fructus aurantii,poria cocos-pinellia,raw rhubarb-Aurantii aurantii,scutellaria scutellaria-pinellia,Radix bupleurum,poria cocos-Tangerine peel,Danshen-pinellia.6.The main core formula combination:(1)tangerine peel,licorice,bamboo Ru,Salvia miltiorrhiza,poria cocos,pinellia pinellia;Bupleurum,scutellaria scutellaria,Astragalus,pinellia pinellia and licorice;(3)Raw rhubarb,trifoliate,magnolia officinalis,sesame seed.Conclusion:1.The main syndromes of constipation in Professor Li Candong’s clinic were large intestine,spleen and liver,and the main syndromes of solid disease were qi stagnation,heat and dampness;The main deficiency syndromes are Yin deficiency,Qi deficiency and blood deficiency.2.The common witness types are liver qi stagnation syndrome,blood deficiency and intestinal dryness syndrome,spleen and stomach qi deficiency syndrome,liver and kidney Yin deficiency syndrome.3.In the treatment of constipation,Professor Li Candong mainly used qi regulating drugs,tonifying deficiency drugs,phlegm-reducing cough and asthmatic drugs,and purging drugs.The drugs were mainly warm,calm and slightly cold drugs,and sweet,octinent and bitter drugs were commonly used.4.Professor Li Candong treated constipation with the principle of "general use of the six fu organs",and used more and more prescriptions such as Wendan Decoction,Xiaobupleurum decoction,Chengqi decoction,Maziren Wan,etc.;Conclusion Constipation takes "depression" and "dryness" as the core pathogenesis,and clinical thinking pays attention to syndrome differentiation and treatment;Patients with liver depression and qi stagnation should conduct qi stagnation;Patients with blood deficiency and intestinal dryness should nourish blood and moisten dryness;Patients with insufficient qi should strengthen the spleen and qi;Patients with liver and kidney Yin deficiency should nourish Yin and promote fluid.
