| Objective:To investigate the incidence of anxiety in patients with atrial fibrillation,and analyze the correlation between traditional Chinese medicine(TCM)syndrome types,symptom severity and other related factors in patients with atrial fibrillation,to understand the distribution of TCM syndrome types in patients with atrial fibrillation complicated with anxiety,and the differences in different TCM syndrome types in patients with atrial fibrillation complicated with anxiety,so as to provide clinical reference for comprehensive prevention and treatment.Methods:A questionnaire survey was conducted by trained investigators and patients with atrial fibrillation to collect the relevant data of the patients.The general information questionnaire,the atrial fibrillation situation questionnaire and the Hamilton Anxiety Scale(HAMA)were filled out.The patients were divided into groups according to the severity of symptoms in patients with atrial fibrillation recommended by the European Heart Rhythm Association using EHRA symptom score.According to the Diagnostic and therapeutic Criteria of palpitation in Traditional Chinese Medicine,the distribution of TCM syndrome types was analyzed,and the correlation between TCM syndrome types,the severity of AF symptoms and other factors in patients with atrial fibrillation and anxiety state was discussed.Results:(1)A total of 132 patients with atrial fibrillation were investigated,including 73 males and 59 females,with a minimum age of 22 years and a maximum age of 79 years.(2)The distribution of TCM syndromes of the selected patients was 45 cases of heart blood stasis(34.09%),32 cases of heart deficiency and timid(24.24%),31 cases of heart and spleen deficiency(23.48%),12 cases of Yin deficiency and fire hyperactivity(9.09%),7 cases of heart Yang deficiency(5.30%),and 5 cases of water qi ling heart(3.79%).(3)The distribution of the severity of atrial fibrillation symptoms was asymptomatic in26 cases(19.70%),mild in 32 cases(24.24%),moderate in 46 cases(34.85%),severe in 28cases(21.21%),and no disability.(4)Gender,severity of AF symptoms and hypertension had statistical significance in predicting anxiety status.(5)The TCM syndrome types of patients with atrial fibrillation combined with anxiety state were as follows: heart blood stasis syndrome > heart deficiency and timid syndrome >heart and spleen deficiency syndrome > Yin deficiency and fire exuberance syndrome > water qi ling heart syndrome > heart Yang deficiency syndrome.The most common type was heart-blood stasis syndrome,accounting for 29.58%.(6)The incidence of anxiety in atrial fibrillation patients with different TCM syndromes was the highest in patients with water-qi ling heart syndrome(100%),and the lowest in patients with heart and spleen deficiency syndrome(35.48%).(7)The HAMA scores of atrial fibrillation patients with different TCM syndromes were compared in pairs,and the HAMA scores of heart and spleen deficiency syndrome,water and qi ling heart syndrome and Yin deficiency and fire Wang syndrome were statistically different,and the other TCM syndromes were not statistically different.(8)The main TCM syndromes of atrial fibrillation patients without anxiety were heart blood stasis syndrome and heart and spleen deficiency syndrome,while the main TCM syndromes of atrial fibrillation patients with anxiety were heart blood stasis syndrome and heart deficiency and timid syndrome.Conclusion:(1)Female,moderate and severe symptomatic AF and hypertension are independent risk factors for AF with anxiety.(2)Heart blood stasis is the most common syndrome in patients with atrial fibrillation,and the constituent ratio of TCM syndromes of atrial fibrillation combined with anxiety from large to small is heart blood stasis syndrome > heart deficiency and timid syndrome > heart and spleen deficiency syndrome > Yin deficiency and fire hyperactivity syndrome > water qi ling heart syndrome > heart Yang deficiency syndrome.(3)The incidence of anxiety was the highest in patients with water-qi lingxin syndrome,while the incidence of anxiety was the lowest in patients with heart-spleen deficiency syndrome.(4)The main TCM syndrome types of atrial fibrillation patients without anxiety were heart blood stasis syndrome and heart and spleen deficiency syndrome.The main TCM syndrome types of atrial fibrillation patients with anxiety were heart blood stasis syndrome and heart deficiency and timid syndrome. |
