Objective:To study the relationship between the changes of cognitive function impairment,facial infrared thermal imaging features and TCM syndrome elements in drug addicts with Yang deficiency before and after the warm moxibustion intervention,so as to provide new ideas for improving cognitive function and evaluating health status of drug addicts during rehabilitation.Methods:From April to September 2022,a total of 120 rehabilitation drug addicts were recruited from 2 drug rehabilitation institutions in Fuzhou City.According to the results of syndrome elements,29 cases were excluded from the disease syndrome elements containing "Yin deficiency" or "heat",and the remaining 91 cases were divided into 2 groups according to whether they contained "Yang deficiency" syndrome elements,among which 44 cases were in the Yang deficiency syndrome element group(Yang deficiency group for short).Non-yang deficiency syndrome element group 47 people(referred to as non-yang deficiency group).Both groups received warm moxibustion intervention 3 times a week for 3 weeks.RBANS test sheet and syndrome differentiation were used to assess cognitive function and traditional Chinese medicine health status before and after intervention in both groups,and facial temperature changes before and after treatment were observed by facial infrared thermal imaging technology.On this basis,the relationship between cognitive impairment and facial infrared temperature change and TCM syndrome elements was analyzed.Results:1.Basic information of drug addicts in recovery: in this study,there were more males than females,aged between 31 and 50,mostly married,generally with low educational level,mostly junior middle school or below,and mostly unemployed.The majority of drug users have been using drugs for more than 10 years,and they are generally treated in drug rehab for1-2 times.Curiosity is the main reason for drug use,accounting for 55.83% of the total number.The relapse rate of drug addicts was very high(98.33%).The main distribution time of relapse was within 1 month and more than 1 year after successful detoxication,and the main reasons for relapse were to kill the boredom,the influence of drug friends and to eliminate the annoyance.2.Distribution characteristics of TCM syndrome elements in drug addicts during rehabilitation period: the distribution frequency of syndrome elements in disease location before warm moxibustion intervention in Yang deficiency group was mainly kidney,spleen,liver,cell palace,stomach,heart and lung from high to low;The distribution frequency of syndrome elements from high to low was mainly Yang deficiency,qi deficiency,phlegm,qi stagnation,blood deficiency,blood stasis,cold,dampness,etc.After intervention,the distribution frequency of disease location syndrome elements from high to low was mainly kidney,heart,liver,lung,spleen,etc.The distribution frequency of syndrome elements from high to low was mainly Yang deficiency,Qi deficiency,phlegm,blood deficiency,qi stagnation and so on.In the non-yang deficiency group,the distribution frequency of disease location syndrome elements was mainly kidney,cell palace and liver from high to low before intervention.The distribution frequency of syndrome elements from high to low was mainly qi stagnation,blood stasis,phlegm,etc.After intervention,the distribution frequency of disease location syndrome elements from high to low was mainly kidney,cell palace,etc.The distribution frequency of syndrome elements from high to low was mainly blood stasis.3.Changes of syndrome element score before and after intervention of drug addicts in rehabilitation stage: after warm moxibustion intervention of Yang deficiency group,disease location and disease syndrome element score decreased significantly;Before and after intervention,the changes of syndrome element score of kidney,liver,spleen,cell palace and lung were statistically significant and significantly decreased(P<0.05),while the changes of syndrome element score of Yang deficiency,qi deficiency,qi stagnation,blood stasis,phlegm and cold were statistically significant and significantly decreased(P<0.05).After the intervention of warm moxibustion in non-yang deficiency group,the scores of disease site and disease syndrome elements decreased.Before and after the intervention,the changes of syndrome element score of cytopoiesis were statistically significant and significantly decreased(P<0.05),and the changes of syndrome element score of qi stagnation and phlegm were statistically significant and significantly decreased(P<0.05).4.The cognitive function of drug addicts in rehabilitation stage and its changes: before intervention,the cognitive function of Yang-deficiency group and non-Yang-deficiency group had statistical significance in delayed memory(P<0.05);And the cognitive function scores of the non-yang deficiency group were slightly higher than those of the Yang deficiency group.There was no significant difference in cognitive function between the Yang deficiency group and the non-yang deficiency group after intervention(P>0.05).The cognitive function scores of both groups improved after intervention,and the scores of the non-yang deficiency group were slightly higher than those of the Yang deficiency group.Cognitive function before and after warm moxibustion intervention in Yang deficiency group was significantly improved in immediate memory,visual span,speech function,attention,delayed memory,total score(P<0.05);The cognitive function before and after warm moxibustion intervention in non-yang deficiency group was significantly improved in immediate memory,visual span,speech function,attention,total score(P<0.05).5.Characteristics and changes of facial infrared temperature of drug addicts in rehabilitation stage: Before intervention,the infrared temperature of Yin Tang and right cheek in Yang deficiency group and non-yang deficiency group had statistical significance,and the temperature of non-yang deficiency group was slightly higher than that of Yang deficiency group(P<0.05).After intervention,there was no significant difference in facial infrared temperature between the Yang deficiency group and the non-yang deficiency group(P>0.05).The changes of facial infrared temperature on forehead,Yintang,right cheek,left temple and whole face before and after warm moxibustion intervention in Yang deficiency group were statistically significant(P<0.05);The changes of facial infrared temperature in Yintang,left cheek and whole face before and after warm moxibustion intervention in non-yang deficiency group were statistically significant(P<0.05).6.The relationship between the changes of cognitive function and the changes of syndrome elements in drug addicts in Yang deficiency group: the immediate memory was positively correlated with liver,spleen,cell palace,qi deficiency,qi stagnation,blood stasis and phlegm(r=0.375,0.323,0.350,0.312,0.407,0.457,0.416,P<0.05);Visual span was positively correlated with cold(r=0.330,P<0.05).Speech function was positively correlated with lung(r=0.378,P<0.05).Note that there was a positive correlation with cell palace(r=0.321,P<0.05).The total score was positively correlated with cell palace,qi stagnation and blood stasis(r=0.386,0.398,0.391,P<0.05).7.The relationship between the changes of cognitive function and facial infrared temperature of drug addicts in Yang deficiency group: The total score of cognitive function was positively correlated with the right cheek(r=0.307,P<0.05).Conclusion:1.The cognitive function level of drug addicts during rehabilitation in Fuzhou area is generally impaired,involving multiple viscera,qi and blood dysfunction,clinically presenting complex syndromes of deficiency and deficiency mixed with deficiency.2.The cognitive function,TCM syndrome elements and facial infrared temperature of drug addicts in the Yang-deficiency group were different from those in the non-yang-deficiency group,and their cognitive level and overall functional state were lower than those in the non-yang-deficiency group.3.The effect of warm moxibustion intervention was more significant in improving the cognitive function and overall state of drug addicts in Yang deficiency group during rehabilitation,and the improvement of cognitive function was positively correlated with the change of infrared temperature of the right cheek,suggesting that facial infrared thermal imaging image features can be used as one of the objective indicators to help reflect the change of cognitive function. |
