Objective: To study the correlation between cystatin C(Cys-C),homocysteine(Hcy)and fibrinogen(FIB)and the degree of coronary artery disease in elderly patients with coronary heart disease,so as to provide a new idea for community serological test indicators to predict the degree of coronary artery disease in elderly patients with coronary heart disease.Methods: A retrospective study was conducted on 385 inpatients over 65 years of age in the Department of Cardiology of Liaozhong District People’s Hospital from July 2020 to July 2022.All patients were suspected of coronary heart disease at first diagnosis and confirmed by coronary angiography.General clinical data and laboratory test results of related patients were analyzed,and coronary heart disease was confirmed according to coronary angiography results.There were 275 patients with stable coronary heart disease in the observation group and 110 patients with non-coronary heart disease in the control group.The observation group was divided into 3 groups according to Gensini score.Among them,109 patients were in the mild stenosis group(0 < Gensini score ≤27),113 were in the moderate stenosis group(27 < Gensini score ≤ 60),and 53 were in the severe stenosis group(Gensini score > 60).The serum levels of Cys-C,Hcy and FIB were compared between the mild,moderate and severe stenosis groups and the control group.Statistical analysis of SPSS26.0 was performed in this paper.The measurement data of patients’ age,serum Cys-C,Hcy and FIB levels were expressed by means of mean plus or minus standard deviation,and T-test was used between the two groups conforming to normal distribution.Data such as the number of patients,gender and history of hypertension were represented by(n,%),Chi-square test was used between the two groups,logsitic regression analysis was used for multivariate analysis,Pearson correlation analysis was used for correlation analysis with different degrees of coronary artery lesions,and ROC curve was drawn using R language to evaluate the value of influencing factors in disease prediction.Results: 1.The comparison of basic data between the observation group and the control group showed that there were significant differences in the indexes of Cys-C,Hcy and FIB between the two groups(P < 0.05);2.Pearson correlation analysis showed that:Cys-C,Hcy and FIB were positively correlated with the degree of coronary artery stenosis(r > 0,P < 0.05).It was found that the sensitivity and specificity of Hcy and Cys-C were 56.3% and 97.5%,respectively,and 58.9% and 90.0%,respectively,in the evaluation of predictive value.The sensitivity and specificity of FIB were 59.4% and87.5%,respectively.Conclusions: The levels of Cys-C,Hcy and FIB are positively correlated with the degree of coronary artery disease in patients with coronary heart disease,and can distinguish the degree of coronary artery disease(mild stenosis,moderate stenosis and severe stenosis),providing a strong basis for clinical evaluation of the degree of disease,diagnosis and treatment.
