Objective: The purpose of this study was to understand the current status of quality of life in patients with gout,to explore the correlation between illness perception,coping style and quality of life in patients with gout,to analyze the influencing factors of quality of life in patients with gout based on stress and coping theory,and to provide a basis for clinical care workers to systematically assess and intervene in the quality of life in patients with gout.Methods: This study used stress and coping theory as the theoretical basis and a cross-sectional study design.By using a convenience sampling method,201 gout patients attending outpatient clinics in a tertiary hospital were recruited as study subjects from October 2021 to March 2022.The study instruments included a general information questionnaire,the Brief Illness Perception Questionnaire,the Medical Coping Modes Questionnaire and the Gout Impact Scale.A database was created and entered into Excel for management,and SPSS 26.0 was applied for data analysis.Count data were described by frequency and percentage;measurement data were described by mean ± standard deviation.One-way analysis of variance(two independent samples t-test,one-way ANOVA)and multi-way analysis of variance(multiple linear regression analysis)were used to analyze the factors influencing the quality of life in patients with gout.The test level was set at 0.05,and P < 0.05(two-sided)indicated that the difference was statistically significant.Results: 1.Current status of quality of life in patients with gout: the total quality of life score was(47.95±10.95),which was at an intermediate level,and the quality of life dimensions with the highest to lowest scores were,in order,“Gout concern overall”,(75.44±16.55),“Gout concern during attack”,(69.47±10.35),“Gout medication side effects”,(53.48±20.24),“Unmet gout treatment need”,(40.09±19.16),“Concern about activity during attacks”,31.25(18.75,43.75),“Well-being during attack”,28.57(14.29,39.29).2.Results of univariate analysis: the differences in the quality of life in patients with gout in terms of occupation,per capita monthly household income,exercise status,disease duration,number of acute gout attacks in the past year,number of hospitalizations for gout in the past year,duration of acute attacks,pain during attacks,tophi status,and blood uric acid values were statistically significant(P < 0.05).3.Results of correlation analysis:(1)illness perception and quality of life in patients with gout were positively correlated(r=0.689,P<0.01);(2)confrontation and quality of life were negatively correlated(r=-0.714,P<0.01),avoidance and submission were positively correlated with quality of life(r=0.278,r=0.338,both P<0.01).4.Results of multifactorial analysis: pain during attacks,number of hospitalizations for gout in the past year,tophi status,blood uric acid values,occupation,illness perception(consequences),illness perception(treatment control),illness perception(emotional response),and coping style(confrontation)entered the regression equation of quality of life in patients with gout,and these variables together explained 80% of the variance level of quality of life in patients with gout.Conclusion: 1.quality of life in patients with gout was at an intermediate level.2.illness perception was positively correlated with quality of life in patients with gout;avoidance and submission were positively connected with quality of life,while confrontation was negatively correlated with it.3.Factors influencing quality of life in patients with gout included pain during attacks,number of hospitalizations for gout in the past year,tophi status,blood uric acid values,occupation,illness perception(consequences),illness perception(treatment control),illness perception(emotional response),and coping style(confrontation).
