Prevalence And Risk Factors Of Atrial Fibrillation Among 986 Patients With Ischemic Stroke In Liaoning Province

Objective:The study aimed to understand the prevalence and characteristics of atrial fibrillation in surviving ischemic stroke patients≥40 years in Liaoning Province,to clarify the awareness and treatment status of atrial fibrillation in patients with ischemic stroke,and to determine the control of other major risk factors in patients with ischemic stroke and atrial fibrillation,in order to provide a basis for the implementation of secondary prevention strategies for stroke and the development of comprehensive management measures for atrial fibrillation in Liaoning Province.Method:From September 2017 to March 2019,a multi-stage stratified cluster random sampling method was used to randomly select 18796 residents≥40 years from 4 counties(Chaoyang,Liaoyuan,Liaoyang,and Donggang)and 3 districts(Gongchangling,Liuerbao,Zhenxing district)in Liaoning Province.A total of 986 eligible patients with surviving ischemic stroke were investigated related to atrial fibrillation,and the prevalence of atrial fibrillation and associated risk factors in these patients were analyzed.Results:The prevalence of atrial fibrillation in 986 surviving ischemic stroke patients≥40years in Liaoning Province was 2.5%,with 3.0%in men and 2.0%in women,with no statistically significant difference(χ~2=0.985,P=0.321).The prevalence was higher in urban areas than in rural areas(4.6%vs 1.8%,P<0.05).The prevalence of atrial fibrillation increased with age,with the highest prevalence(6.4%)in patients≥80 years.The awareness rate of atrial fibrillation was 80.0%,with 86.7%in men and 70.0%in women,and 91.7%in urban areas and 69.2%in rural areas.The treatment rate of atrial fibrillation was 8.0%,with 13.3%in men and 0%in women,and 16.7%in urban areas and 0%in rural areas,with no statistically significant differences(P>0.05).The prevalence of hypertension,diabetes,and abnormal blood lipid levels was 84.0%,40.0%,and 52.0%in patients with atrial fibrillation,and the prevalence of current smoking,current drinking,lack of exercise,and being overweight or obese was 20.0%,28.0%,28.0%,and 72.0%,respectively.The rate of awareness,treatment and control of hypertension were 81.0%,71.4%,and 4.8%,respectively,the rate of awareness,treatment and control of diabetes were 70.0%,60.0%,and 33.3%,respectively,and the rate of awareness,treatment and control of dyslipidemia were 38.5%,30.8%,and 23.1%,respectively.Conclusion:The prevalence of atrial fibrillation in 986 surviving ischemic stroke patients≥40 years in Liaoning Province is 2.5%,which was seriously insufficient in treatment,complicated with numerous risk factors,high prevalence of chronic diseases,but low control rate.Therefore,it is imperative to strengthen the comprehensive management of atrial fibrillation patients with ischemic stroke and implement long-term effective intervention measures against major risk factors to avoid the risk of recurrent stroke,reduce the mortality and disability rate of patients,and reduce the burden of disease.
