| Objective:The disease composition of inpatients in the department of general medicine in a tertiary hospital in Liaoning Province from 2014 to 2019 was studied to provide a reasonable reference basis for the discipline construction of general medicine in this hospital and other general hospitals in the early stage of development,such as the training of general practitioners,the improvement of general medical services,the development strategy of general medicine,the setting of general medicine wards,and the allocation of medical resources.Methods:Inpatients of the Department of Family Medicine of a tertiary care hospital in Liaoning Province from 2014-2019 were used as the study population.Patients’ information including age,gender,length of stay,days of hospitalization,type of visit,and diagnosis were collected through the hospital’s case management system,and the statistical summary was performed by the first diagnosis in the first page of the inpatient case according to the international disease classification coding principle,and patients’ disease diagnoses were classified according to the international disease coding principle ICD-10.The age of inpatients is divided into ≤ 45,46-68,and ≥ 69 years old.The number of days in hospital was divided into ≤7 days,8-14 days,and ≥15 days.The number of illnesses was divided into 1,2-8,and ≥ 9.The number of illnesses was classified as 1,2,and ≥ 3.The collected data were retrospectively analyzed by using Excel16 for data entry and SPSS 26.0 statistical software for data analysis,and the count data were expressed as percentages using statistical methods such as the χ2 test,with p<0.05 as a statistically significant difference.Results:The total number of inpatients in the Department of Family Medicine in a tertiary care hospital in Liaoning Province from 2014-2019 was 8682,of which 3224(37.13%)were male patients and 5458(62.87%)were female patients,with a male to female gender ratio of 100:169;the number of inpatient cases increased year by year from 2014-2018,with a growth rate of-22.94% in 2019(1562 cases)compared to 2018(2027 cases)with a growth rate of-22.94%;the age range of inpatients was 16-104 years,with an average age of(60.9 ± 12.35)years;the total number of cases in the top 10 inpatient rankings from 2014 to 2019 was 7090,accounting for 86.65% of the total number of all inpatients,and the top five in the ranking of disease composition were more stable in order of coronary artery atherosclerotic heart disease,The top five in the disease system classification were circulatory system diseases,neurological system diseases,endocrine system diseases,digestive system diseases,and respiratory system diseases;the Pareto chart suggested that circulatory system diseases,neurological system diseases,endocrine system diseases,and respiratory system diseases in the disease system classification were in the top five,neurological system diseases,endocrine system diseases,and digestive system diseases are the most dominant,and the remaining systems are secondary;from 2014 to 2019,the shortest number of days patients were hospitalized was 1 day,the longest was 46 days,and the total average hospitalization days was 6.45 days,with a median of 7 days;the longest average hospitalization days of11.79 days occurred in 2014,and the shortest average hospitalization days of 5.06 days occurred in 2018,with statistically significant differences in the number of hospitalization days across years(p < 0.05);The age percentage of inpatients changed from 2014 to 2019,and the age of inpatients in each age group ≤45 years old did not correlate with the change of year,the number of cases of patients aged 46-68 years old and ≥69 years old showed an increasing trend with the year(p trend < 0.05),with an average annual growth rate of 1.04% and 0.91%,respectively;coronary atherosclerotic heart disease in the age groups ≤ 45 years old,The percentages of coronary atherosclerotic heart disease in each age group were much higher than those in the second place in each age group,and the percentages of patients with coronary atherosclerotic heart disease in the age group ≤45 years old were relatively the lowest,accounting for 29.9%,2155 cases accounting for 44.01%,and 891 cases accounting for43.72%,respectively.The median number of hospitalizations was 1,the plural number was 1,and the mean number was 1.72,and the total number of hospitalizations was 6371,accounting for 73.38% of the total number of hospitalizations.The median number of co-morbidities was 8,the plural number was 7,and the mean number of co-morbidities was 7.81.The majority of patients in each age group had co-morbidities,and the total number of co-morbidities was 8604 cases,accounting for 99.10% of all hospitalized patients,and the number of co-morbidities varied significantly with age.Conclusion: In terms of disease composition and disease system classification,the top five disease compositions are relatively stable,while the bottom five disease compositions are relatively variable.The top ten diseases account for more than 80% of hospitalizations,indicating that the top ten diseases are the main reason for hospitalization.The top four diseases in the disease system classification were the most important factors influencing disease classification.The overall distribution of the disease composition of inpatients varied by age,year,gender,and other conditions.The average length of stay of patients showed a yearly decrease basically and stabilized between 5 and 6 days.Patients of all ages were predominantly co-morbid,accounting for more than ninety percent,and the number of co-morbidities gradually increased with increasing age,with co-morbidity becoming more common. |
