| Objective:To develop a prenatal depression screening scale for prenatal depression screening and provide better tools for clinical and scientific research to evaluate prenatal depression;Prenatal depression screening scale was initially applied to investigate the status quo of prenatal depression of pregnant women,explore the factors affecting prenatal depression of pregnant women,and provide the basis for early detection and early intervention of perinatal depression,and reduce the adverse outcome of perinatal depressionMethods:(1)Based on the diagnostic criteria for depression in the Diagnostic and Statistical Manual of Mental Disorders(DSM-5)and the International Classification of Diseases(ICD-11),as well as various types of depression self-assessment scales,a large number of literatures related to prenatal depression were retrieved,and structured interviews were conducted with several perinatal pregnant women to establish an entry pool.(2)After expert consultation,delete and modify the entries according to expert opinions.(3)35 pregnant women were selected from the obstetric clinic to conduct a preliminary survey.The words used in the questionnaire were popularized,and the items unsuitable for pregnant women were further deleted,and the scale was initially formed.Using convenient sampling to conduct a formal survey in the outpatient department of obstetrics.(5)SPSS and Amos were used to screen the items and test the reliability and validity of the scale,and finally the prenatal depression scale was formally determined.(6)Pregnant women were investigated with the General Survey Scale,Prenatal Depression Screening Scale,Edinburgh Postnatal Depression Scale,Beck Depression Scale,PHQ-9,Hamilton Depression Scale,to explore the factors affecting the score of the Prenatal Depression Screening Scale,and to draw the ROC curve of each scale,compare the area under the curve,and select the most suitable depression evaluation scale for pregnant women in combination with clinical practice.Results:(1)Expert consultation results: the enthusiasm of experts is 100%,the authority coefficient(Cr)is 0.810,and the items are modified under the advice of experts.(2)Through frequency analysis method,coefficient of variation method,high and low grouping method,total correlation coefficient method,Cronbach’s α coefficient analysis and exploratory factor analysis showed that the factor load of each item was greater than 0.4,and the overall cumulative variance contribution rate was 65.408%.Scale total Cronbach’s α coefficient is 0.924,the retest reliability is 0.880,the split-half reliability is 0.873,and the reliability is good.The confirmatory factor analysis showed that χ/df=1.461,RMSEA=0.064,CFI=0.920,GFI=0.869,TLI=0.901,IFI=0.923,the fitting indicators were up to standard,and the structural validity was good.Finally,the scale includes 16 items and 4 dimensions,including 6 items for depressive symptoms,4 items for negative evaluation,3 items for anxiety symptoms and 3 items for social support.(3)Taking the general condition of the patient as the independent variable,and the total score of prenatal depression screening as the dependent variable,the univariate analysis of variance was performed respectively.The statistically significant differences of each variable grouping were "family monthly income","personal income","planned pregnancy and expected value","complications during pregnancy","history of depression","premenstrual irritability history","delivery medical insurance" "History of dysmenorrhea".(4)The area under the EPDS curve is 0.864,PHQ-9 is 0.871,Beck is0.907,and the prenatal depression screening scale is 0.877,second only to Beck,but in clinical practice,the prenatal depression screening scale is more suitable for pregnant women.Conclusion:(1)The preparation of the Prenatal Depression Screening Scale is in line with the basic process of the scale preparation,and its reliability and validity are up to standard.It can be used for prenatal depression screening.(2)Pregnant women often feel less agile,irritable,anxious and poor sleep quality during pregnancy.(3)Economic level,complications during pregnancy,history of depression,history of premenstrual irritability,and history of dysmenorrhea are the influencing factors of prenatal depression.(4)Compared with EPDS,HAMD,PHQ-9 and Beck,this scale is more suitable for prenatal depression screening of pregnant women. |
