| Objective: Tonsillectomy is one of the oldest and most common operations in otorhinolaryngology, which can improve the symptoms of childhood snoring caused by tonsillar hypertrophy.Although the surgical technique and difficulty are not high, and the wide application of electric knife and low-temperature plasma knife in recent years has greatly improved the efficiency and safety of surgery, complications such as infection, pain and bleeding still occur from time to time, which affect the postoperative recovery of children, and even endanger the life of children in severe cases. In recent years, the influence of the wide use of antibiotics on postoperative complications has attracted the attention of many scholars at home and abroad, and has achieved fruitful results. However, the influence of administration methods on postoperative complications still needs to be studied and explored. Therefore, this study aims to evaluate the influence of different administration methods of antibiotics on postoperative complications after tonsillectomy,so as to provide a basis for the application of antibiotics after tonsillectomy. Objective:tonsillectomy is one of the oldest and most common operations in otorhinolaryngology, which can improve the symptoms of chronic tonsillitis and childhood snoring caused by tonsillar hypertrophy.Although the surgical technique and difficulty are not high, and the wide application of electric knife and low-temperature plasma knife in recent years has greatly improved the efficiency and safety of surgery, complications such as infection, pain and bleeding still occur from time to time, which affect the postoperative recovery of children, and even endanger the life of children in severe cases. Methods: A total of 223 children who underwent tonsillectomy with or without adenoidectomy in the Department of Otorhinolaryngology,Shengjing Hospital Affiliated to China Medical University from January 2021 to December 2022 were collected.According to the method of postoperative antibiotic administration, they were divided into two groups: oral group(80 cases)and intravenous group(143 cases). The incidence of postoperative bleeding, pain and other complications were observed and compared between the two groups.SPSS 23.0 software was used for statistical analysis and comparison between the two groups. Results: There was no significant difference in postoperative bleeding rate between oral group and intravenous group(P=0.6413 > 0.05).The daily pain was compared between the two groups. There were significant differences in the degree of pain between the two groups 1-3 days after operation(P< 0.05).There was no significant difference in the degree of pain between the two groups on the 4th day after operation(P=0.1472 > 0.05).The average pain degree of the two groups was compared, too. The average pain score of the oral group within 4 days after operation was 4.71±1.4062.The average pain score of the intravenous group was 5.24±1.3199,and the difference between the two groups was statistically significant(P=0.0056 < 0.05).There was no significant difference in other complications between the two groups(P > 0.05).The time of complete shedding of tunica albugica in oral group was 15.04±1.912,and that in intravenous group was 15.92±1.778,and the difference between the two groups was statistically significant(t=3.444,P < 0.05).Factors related to the degree of pain include age, weight, tonsil division and the use of antibiotics. At the same time, the degree of pain is positively correlated with the time of tunica albuginea shedding. Conclusion: Oral antibiotics are better than intravenous antibiotics in relieving pain and reducing other complications within 4 days after operation. It can shorten the time of complete loss of tunica albuginea of tonsil fossa after operation. There was no significant difference in the rate of postoperative bleeding and other complications between oral and intravenous antibiotics. |
