Objective: Using optical coherence tomography(OCT)to get the characteristics of the choroidal thickness and the Choroidal Vascularity Index(CVI)change in hypertensive Patients,aiming to analyze the relationship between them and systemic factors.Methods: This study is a cross-sectional observational study.The observation objects of this study were 130 hypertensive Patients who were treated in the hyperten sion center from may 2019 to June 2020.All patients underwent the anthropometric examination,serological and biochemical blood investigations,intraocular Pressure,fundus Photography,OCT examination.After setting the scales,the 500-μm superior choroidal thickness were measured and recorded as SCT.The 500-μm inferior choroidal thickness were measured and recorded as ICT.The 500-μm nasal choroidal thickness were measured and recorded as NCT.The 500-μm temporal choroidal thickness were measured and recorded as TCT.The Subfoveal choroidal thickness were measured too.Then use Image-J software to turn images into binarized images,then measure the LA(Luminal area,LA),SA(Stroma area,SA)and TCA(Total choroidal subfoveal area,TCA),and calculate CVI of each area.Results: A total of 130 Patients(130 eyes)were included(70 male,60female),In the uni-variate analysis,SCT were negatively correlated with age(β=-1.494,95%CI[-2.299--0.689],P<0.001),SCT negatively correlated with HDL-C(β=-52.996,95%CI[-94.096--11.896],P=0.012),SCT were positively correlated with Ipsilateral internal carotid artery(β=27.138,95%CI[10.163-44.114],P=0.002),SCT were negatively correlated with systolic pressure(β=-0.796,95%CI[-1.487--0.105],P= 0.024);ICT were negatively correlated with age(β=-1.513,95%CI[-2.334--0.692],P<0.001),ICT were negatively correlated with Ipsilateral internal carotid artery(β=21.629,95%CI [4.078-39.181],P=0.016),NCT were negatively correlated with age(β=-1.385,95%CI [-2.211-0.559],P=0.001),NCT positively correlated with Ipsilateral internal carotid artery(β=19.351,95%CI[1.959-36.742],P=0.029),NCT were negatively correlated with systolic pressure(β=-0.836,95%CI[-1.538--0.134],P=0.020),NCT were negatively correlated with HDL-C(β=-54.450,95%CI [-96.014--12.886],P=0.011);TCT were negatively correlated with systolic Pressure(β=-0.990,95%CI[-1.653--0.327],P=0.004),TCT were negatively correlated with age(β=-1.250,95%CI[-2.043--0.457],P=0.002),TCT were negatively correlated with HDL-C(β=-47.924,95%CI [-88.110--7.738],P=0.020),TCT positively correlated with Ipsilateral internal carotid artery(β=20.364,95%CI[3.813-36.915],P=0.016);SFCT were negatively correlated with systolic Pressure(β=-0.821,95%CI[-1.507-0.134],P=0.020),SFCT were negatively correlated with age(β=-1.193,95%CI[-2.008-0.377],P=0.004),SFCT were negatively correlated with HDL-C(β=-53.652,95%CI[-94.207--13.098],P=0.010).SFCT were Positively correlated with Ipsilateral internal carotid artery(β=25.016,95%CI[8.144-41.888],P=0.004);Choroidal Vascularity Index were negatively correlated with systolic Pressure(β=-0.047,95%CI[-0.083--0.012],P=0.008),Choroidal Vascularity Index were negatively correlated with age(β=-0.045,95%CI[-0.088--0.002,P=0.038),choroidal Vascularity Index were negatively correlated with hypertension medical history(β=-0.101,95%CI[-0.188--0.013],P=0.024),choroidal Vascularity Index were negatively correlated with Total cholesterol(β=-0.931,95%CI[-1.672--0.190],P=0.014),The differences were all significantly.In the multiple regression analysis,Choroidal Vascularity Index were negatively correlated with Total cholesterol(β=-0.812,95%CI[-1.580--0.045],P=0.038),SCT were positively correlated with Ipsilateral internal carotid artery(β=24.200,95%CI[7.973-40.427],P=0.004),NCT were positively correlated with ipsilateral internal carotid artery(β=16.703,95%CI[0.240-33.166],P=0.047),NCT were negatively correlated with systolic pressure(β=-0.725,95%CI[-1.391--0.059],P=0.033);TCT were positively correlated with Ipsilateral internal carotid artery(β=16.620,95%CI [0.777-32.463],P=0.040);TCT were negatively correlated with systolic pressure(β=-0.862,95%CI[-1.503--0.222,P=0.009);SFCT were positively correlated with Ipsilateral internal carotid artery(β=22.566,95%CI[6.465-38.667],P=0.006)SFCT were negatively correlated with systolic pressure(β=-0.699,95%CI [-1.351--0.048],P=0.035).Conclusion:(1)The change of Choroidal Vascularity Index and choroidal thickness is associated with the risk factors of cardiovascular disease related to hypertension.(2)The increase of Serum total cholesterol level will be accompanied by the decrease of choroidal vascularity index in hypertensive patient.(3)The increase of ipsilateral internal carotid artery will be accompanied by the increase of 500-μm superior choroidal thickness,500-μm nasal choroidal thickness,500-μm temporal choroidal thickness and subfoveal choroidal thickness in hypertensive patient.(4)The increase of systolic Pressure will be accompanied by the decrease of 500-μm nasal choroidal thickness,500-μm temporal choroidal thickness and subfoveal choroidal thickness in hypertensive patient.
