Objective: Lower extermity varicose veins are one of the common diseases in vascular surgery.It is generally believed that there is aseptic inflammatory reaction in the inner wall of lower limb blood vessels,and the mechanism of foam hardener also depends on its stimulation on vascular endothelial cells to induce inflammatory response.The inflammatory cytokines tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)are often involved in the process of human inflammatory response.We used immunohistochemistry to study the expressions of TNF-α and IL-6 in vascular tissue without obvious varicose(inner diameter < 3mm),varicose vascular tissue(inner diameter > 5mm)and varicose vascular tissue injected with foam hardener in the great saphenous vein of patients with varicose veins.We discuss their function and correlation,verified the mechanism of foam hardener,and provide reference for follow-up research and clinical diagnosis and treatment.Methods: Thirty patients with lower extermity varicose veins who needed surgical treatment receive treatment in our hospital from May 1,2022 to September 30,2022 were selected.No obvious varicose vascular tissue,the varicose vascular tissue and the varicose vascular tissue after injection of sclerotherapy was taken from the great saphenous vein of each patient during surgery,then made into pathological sections and immunohistochemical staining of TNF-α and IL-6 were performed,then observed and analyzed under 400 x optical microscope.SPSS 25.0 statistical software was used for all statistical processing.c2 test was used to compare rates,and P< 0.05 was considered statistically significant.Pearson correlation analysis was used for correlation analysis.Results: The positive rates of TNF-α were 16.7%,53.3% and 80.8% respectively in the non-obvious varicose vascular tissue,the varicose vascular tissue and the varicose vascular tissue injected with foam hardener.The difference in the positive rates of TNF-α in the non-obvious varicose vascular tissue and the varicose vascular tissue was statistically significant(P=0.003).There was significant difference in the positive ratebetween varicose vascular tissue and the varicose vascular tissue injected with foam hardener(P=0.031).The positive rates of IL-6 were 20.0%,46.7% and 73.1% respectively in the non-obvious varicose vascular tissue,the varicose vascular tissue and the varicose vascular tissue injected with foam hardener.The difference in the positive rates of IL-6 in the non-obvious varicose vascular tissue and the varicose vascular tissue was statistically significant(P=0.028).There was significant difference in the positive rate between varicose vascular tissue and the varicose vascular tissue injected with foam hardener(P=0.045).By Pearson correlation analysis,the expression of TNF-α and IL-6 in varicose vascular tissues was correlated at 0.008 significance level,and the correlation coefficient was 0.473(positive correlation).The expression in the hardener varicosity vascular tissue was correlated at the significance level of 0.068,and the correlation coefficient was 0.364(positive correlation).Conclusion: The The expressions of TNF-α and IL-6 in varicose veins were significantly higher than those non-obvious varicose veins,which means there was an inflammatory reaction involving TNF-α and IL-6 in the inner wall of varicose veins.The positive rates of TNF-α and IL-6 in varicose veins were significantly higher after injection with foam hardener than those varicose vascular tissue without injection,which aggravated the inflammatory reaction in varicose veins.The expressions of the two inflammatory factors were positively correlated in varicose vascular tissue and the varicose vascular tissue injected with foam hardener,TNF-α was correlated with IL-6 in the occurrence and development of the disease. |
