Objective: Through information extraction of patients’ inpatient medical records and analysis by statistical software,the factors affecting the effect of drug reversion in patients with atrial fibrillation were obtained,the key points of pharmaceutical care in patients with atrial fibrillation were discussed,the path of pharmaceutical care for patients with atrial fibrillation was established,and the path of pharmaceutical care was implemented in inpatients in the department of cardiology to verify the implementation effect of the path.Methods: HIS system was used to look up patients who were discharged from hospital and diagnosed with atrial fibrillation within the whole year of 2020.The patient case information was extracted,and the main factors affecting the drug conversion effect were obtained by using univariate Chi-square test and multivariate logistic analysis.Combined with the analysis results of influencing factors,the focus of pharmaceutical care for patients with atrial fibrillation was discussed,the focus of medication education was adjusted,the contents and methods of medication monitoring were clarified according to clinical guidelines and relevant literature,the path of pharmaceutical care for patients with atrial fibrillation was developed,and the appropriate number of samples of patients with atrial fibrillation were selected for pharmaceutical care according to the inclusion and exclusion criteria,and the patients with atrial fibrillation who did not receive pharmaceutical care were selected as the control group.The effect of clinical pharmacy pathway was evaluated from the aspects of hospital stay,economic cost and treatment satisfaction.Patients were followed up for 4 to 6 months after discharge,and medication compliance of patients was obtained while medication was monitored on time.Statistical analysis was conducted to evaluate the implementation effect of pharmaceutical care pathway.Results: The medical records of patients were extracted and statistical analysis showed that gender,history of arrhythmia greater than 5 years,smoking history,drinking history,accompanying myocardial infarction,accompanying infection,accompanying hypertension,accompanying cancer,and history of statin use had no significant statistical difference between the two groups(p>0.05),while age,history of arrhythmia,History of arrhythmia greater than one year,smoking age greater than 20 years,concomitant coronary heart disease,recent well controlled blood pressure,admission blood glucose higher than7.8,left atrial enlargement,ejection fraction lower than 60,serum magnesium ion concentration,and other factors had a statistically significant difference between the two groups(p<0.05).The results of multi-factor logistics regression showed that age and past history of arrhythmia were independent factors influencing the effect of drug conversion sinus rhythm.Inpatients with atrial fibrillation from April to June 2022 were selected as the control group,and inpatients with atrial fibrillation from July to September 2022 were selected as the intervention group for the implementation of pharmaceutical care pathway.After comparing the data of the intervention group and the control group,it can be concluded that the length of hospital stay and hospitalization cost were reduced to a certain extent after pharmaceutical care was provided to patients according to the pharmaceutical care pathway.Follow-up results showed that the compliance of the intervention group was higher than that of the control group,which proved the feasibility and effectiveness of pharmaceutical care pathway in clinical implementation.Conclusion: A review and analysis of the medical record information of patients with atrial fibrillation showed that the factors affecting drug conversion sinus rhythm of patients with atrial fibrillation were the factors affecting drug conversion sinus rhythm of patients with atrial fibrillation.A feasible and effective pharmaceutical care path was developed from the perspective of clinical pharmacists combined with clinical guidelines,which could bring obvious benefits in the management of patients with atrial fibrillation during their hospitalization.The improvement of compliance can also show the benefits of pharmaceutical care in the long-term management of patients. |
