| Objective: To investigate the efficacy and safety of mechanical thrombectomy using EmboTrap Ⅱ stent in the treatment of acute ischemic stroke of anterior circulation large vascular occlusion Method:Patients at the department of neurology in the Fourth Affiliated Hospital of China Medical University from September 2020 to June 2022 have been retrospectively analysed.All of them were acute ischemic stroke caused by anterior circulation large artery occlusion confirmed by DSA,CTA or MRA.We recorded the basic information of the patients,including age,gender,past medical history,smoking history,history of alcohol,scale of NIHSS.The patients were treated by thrombectomy stent combined with the distal access catheter.The patients were divided into two groups according to different thrombectomy stents,EmboTrap Ⅱ thrombectomy stent was used as the observation group(E group)and Solitaire thrombectomy stent was used as the control group(S group).We also recorded the medical information of the patients,including score of modified Rankin scale(mRS)at 90 days,the rate of recanalization,the rate of first recanalization,times of mechanical thrombectomy,the rate of mortality in perioperative period,the rate of symptomatic intracranial haemorrhage(sICH),the time from puncture to recanalization and time from onset to recanalization.The primary effective outcome was the rate of good prognosis.Good prognosis was defined as a score of 0 to 2 on the modified Rankin scale at 90 days.The secondary effective outcome was the rate of first recanalization.The primary safety outcomes was the rate of mortality in perioperative period.The secondary safety outcome was the rate of sICH.SPSS26.0 software was used to analyze the above data.Result: 69 eligible patients were included.There were 33 patients in the E group and 36 patients in S group.The patients aged 25-88 years(mean 64 ±12)were included in this study.There were no significant differences in age,gender,past medical history,smoking history,history of alcohol,scale of NIHSS between E group and S group(P>0.05).The primary effective outcome was the rate of good prognosis.The rate of good prognosis of E group was higher than S group(48.5% vs 25.0%,P=0.043).The secondary effective outcome was the rate of first recanalization.The rate of first recanalization of E group was higher than S group(79.7% vs 44.4%,P=0.035).The primary safety outcome were mortality in perioperative period,there was no significant difference between the two groups.The secondary safety outcome was sICH,there was no significant difference between the two groups(P>0.05).In addition,compared with S group,Time from puncture to recanalization of E group was shorter(67.27±26.31 vs 115.94±54.67 min,P=0.000),the time from onset to recanalization of E group was shorter(366.06 ± 144.84 vs 695.08 ± 697.84 min,P=0.009).Conclusion: Compared with Solitaire stent,mechanical thrombectomy using EmboTrap Ⅱ stent can improve the prognosis of patients and has a high rate of first recanalization rate in the treatment of acute ischemic stroke of anterior circulation large vascular occlusion,and there was no significant difference between the two groups. |
