| Objective:To explore the correlation between TEG and severe multiple injury patients complicated with MODS,so as to provide theoretical basis for clinical diagnosis and treatment.Methods:The clinical data of 71 patients with severe multiple injuries admitted to the emergency center of our hospital from January 2020 to December 2022 were retrospectively analyzed,and they were divided into MODS group(n=20)and non-MODS group(n=51)according to the occurrence of MODS in the course of treatment.The key clinical indicators such as blood routine,coagulation function and TEG were recorded.The differences of various indexes between the two groups were compared,and the possible risk factors for MODS in patients with severe multiple injuries were screened.Correlation analysis was used to explore the correlation between multiple indexes of TEG,between TEG and the above biochemical indexes.And use the ROC curve to investigate the value of TEG in the diagnosis of MODS in patients with severe multiple injuries.Results:Multivariate regression analysis showed that higher ISS score,increased K value and increased D-dimer were independent risk factors for MODS in severe multiple injuries(P<0.05).The K value of MODS group was significantly higher than that of non-Mods group,and the MA value was significantly lower than that of non-Mods group(P<0.05).R-value was positively correlated with APTT and FIB(P<0.01),was negatively correlated with D dimer(P<0.05),K value was positively correlated with D dimer(P<0.05),α Angle was negatively correlated with PT and D dimer(P<0.05),MA value was positively correlated with platelet and FIB value(P<0.05).ROC curve analysis showed that K value in the TEG could be used to diagnose MODS in patients with severe multiple injuries,and the AUC was 0.796(95%CI 0.686-0.907),while the diagnostic ability of the other three indexes was limited.The combined diagnostic ability of the four indexes was stronger than that of the single indexes,with the AUC of 0.826(95%CI0.733-0.920),sensitivity of 100% and specificity of 62.7%(P<0.05).Conclusion:The increase of ISS score,K value and D-dimer are independent risk factors for MODS in patients with severe multiple injuries.The K value of TEG has a certain value index for the diagnosis of MODS in patients with severe multiple injuries,and the combined efficacy of the four indexes is better than that of the single index. |
