Objective: To investigate the application value of noninvasive prenatal DNA(NIPT)in the screening of chromosome aneuploidy in IVF singletons and natural pregnancies.Methods: We collected 15693 pregnant women who underwent NIPT screening at the Reproductive Medicine Center of the First Affiliated Hospital of Xinjiang Medical University from January 2020 to June 2022,including 1574 cases in the IVF group and14119 cases in the natural pregnancy group,and retrospectively analyzed the NIPT high-risk and prenatal diagnosis results to compare the two groups in terms of T21,T18,T13,SCA,other autosomal aneuploidy The differences in the positive predictive values of T21,T18,T13,SCA,other autosomal aneuploidy,and chromosomal microdeletions/microduplications between the two groups were compared.Results: The differences between the two groups were statistically significant(P < 0.05)when comparing the basic conditions of age and gestational week,more pregnant women in the IVF group were ≥35years old than in the natural pregnancy group,and the gestational week was earlier than in the natural pregnancy group.the sensitivity of NIPT screening for T21,T13,SCA,other autosomal aneuploidies,and chromosomal microdeletions/ microduplications in the IVF group was 100%,100%,100%,100%,and The sensitivity of NIPT screening for T21,T18,T13,SCA,other autosomal aneuploidy and chromosomal microdeletion/ microduplication in the control group was 100%,100%,100%,100% and 100%,respectively,and the specificity was 100%,100%,99.53%,99.79% and 100%,respectively,and the positive predictive value was 100%,100%,66.67%,77.78% and 100%,respectively.The sensitivity was 100%,100%,100%,100%,100%,100%,100%,and specificity was 99,68%,99.64%,98.84%,98.37%,98.59%,98.94%,and positive predictive values were88.46%,72.73%,52.38%,70.00%,54.00%,58.33%,respectively.The comparison between the positive predictive values of NIPT in screening for T21,T18,T13,SCA,other autosomal aneuploidies,and chromosomal microdeletions/ microduplications was not statistically significant between the two groups(P > 0.05).Conclusion: NIPT has the same high accuracy and specificity for screening fetal chromosomal aneuploidy in IVF pregnancies as in natural pregnancies,especially for T21,and therefore can be used as a screening technique for common fetal chromosomal aneuploidy disorders.However,there are certain false positives,which cannot replace invasive prenatal diagnosis.Adequate explanation by clinicians before the application of NIPT is the key to rational and correct application of NIPT.
