Objective:To investigate the clinicopathological features,treatment and prognosis of breast fibroadenoma(CWFA).Methods : 1)120 cases of breast fibroadenoma were examined by ultrasonography and mammography,and were compared with those of CWFA and mammography.2)60 cases of breast fibroadenoma were collected from January 1,2010 to December 31,2022 in the affiliated Tumor Hospital of Xinjiang Medical University.The general conditions,surgical methods,pathological characteristics,postoperative treatment and OS(overall survival),DFS(Disease-free survival)at 3,5,10 years after operation were analyzed.Kaplan-meier method was used to draw survival curves to describe OS and DFS.Cox proportional hazards model was used to analyze univariate and multivariate analysis of prognostic factors.Results:1)the results showed that there were significant differences in age distribution,b-ultrasound boundary,b-ultrasound Echo,molybdenum target shape and molybdenum target boundary between the two groups(p < 0.05).The average age of patients with breast fibroadenoma was46.70 ± 9.41 years old,the median age was 46(39 ~ 53)years old,most of them were over40 years old.The average age of patients with breast fibroadenoma was 42.80 ± 7.09 years old,the median age was 43(39.47)years old.There was no significant difference in age between the two groups.2)modified radical resection was the main method of operation,accounting for 70.00%(42/100).3)the size of tumor was less than 2 cm in 43 cases(71.67%),larger than 2 cm in 17 cases(28.33%).4)the pathological stages were stage I,II in 28 cases(46.67%),III in 4 cases(6.67%).5)the immunophenotype of ER/PR,Her-2 and Ki-67 were positive in all patients,and axillary lymph metastasis in 20 cases(33.33%).The results showed that 60 patients were followed up for 7 ~ 145 months,with a median follow-up of 70(44 ~ 89)months and an average follow-up of 69.62 ± 36.61 months.The mean survival time of OS was 138.48 ± 3.62 months,and that of DFS was120.74 ± 6.50 months.Univariate analysis showed that tumor size > 2 cm,lymph node metastasis,pathological stage II and III,ER/PR(+),Her-2(+),Ki-67(+),CK5/6(+)were significantly associated with poor prognosis(p < 0.05).Multivariate regression analysis showed that lymph node metastasis and CK5/6(+)were significantly associated with poor outcome(p <0.05).Conclusion:The age of patients with breast fibroadenoma is older.Ultrasonography and mammography are valuable in the differential diagnosis of CCFA and FA.Tumor size > 2 cm,lymph node metastasis,pathological stage II and III,ER/PR(+),Her-2(+),Ki-67(+),CK5/6(+)were the influencing factors of poor prognosis,lymph node metastasis and CK5/6(+)were the independent risk factors of poor prognosis.The number of death cases was less and the survival rate was better.Postoperative chemotherapy,radiotherapy and endocrine therapy can reduce the risk of recurrence and metastasis. |
