Purpose:Explore the FFA level of patients with type 2 diabetes,and further analyze the factors that affect the FFA level of T2DM patients,and provide a basis for comprehensive diagnosis and treatment of T2DM.Method:Selected type 2 diabetes patients who attended the Endocrinology Department of the First Affiliated Hospital of Xinjiang Medical University from January to December 2020 as the DM group,and non-diabetic patients as the control group.The basic clinical physical examination data and blood biochemical indicators were collected.Results:(1)A total of 718 cases(male48.6%)were enrolled in the DM group,with an average age of(52.07±10.35)years and an FFA level of(0.42±0.18)mmol/L.A total of 218 cases(male 55.8%)were included in the control group,with an average age of(52.50±11.95)years old,and FFA level(0.34±0.18)mmol/L.(2)According to BMI,it is divided into three layers:BMI<24kg/m~2,24≤BMI<28kg/m~2and BMI≥28kg/m~2;the FFA of the DM group is higher than that of the control group(FFA level:BMI<24kg/m~2(0.40 vs 0.29mmol/L);24≤BMI<28kg/m~2(0.41vs 0.34mmol/L),BMI≥28kg/m~2(0.45 vs 0.41mmol/L)(P<0.01).The FFA level of the DM group and the control group increased with BMI And gradually increased.(3)The subjects in the DM group were divided into three groups according to the course of DM(the course of DM<5 years group FFA(0.45±0.18)mmol/L,the course of 5≤DM<15years group FFA(0.40±0.17)mmol/L,the course of DM≥15 years FFA(0.37±0.17)mmol/L,P<0.01);(4)Divided into three groups according to Hb A1c level(Hb A1c<7%group FFA(0.41±0.16)mmol/L,7%≤Hb A1c<9.5%group FFA(0.41±0.17)mmol/L,Hb A1c≥9.5%group FFA(0.43±0.20)mmol/L,P>0.05).(5)Divide the DM group into five groups according to the number of complications and complications(hypertension,coronary heart disease,positive microalbuminuria,peripheral neuritis,diabetic retinopathy):no complication group,1 complication group,and 2 complications Group 3complication groups and 4 complication groups,(uncomplicated FFA level 0.44(0.41-0.48)mmol/L,combined 1 complication FFA level 0.42(0.40-0.45)mmol/L,combined 2 complications FFA level of 0.41(0.39-0.44)mmol/L,combined 3complications FFA level 0.39(0.37-0.42)mmol/L,combined 4 complications FFA level0.40(0.34-0.47)mmol/L,P>0.05).(6)Among the different hypoglycemic drug treatments,the insulin+biguanide treatment group had the lowest FFA level,followed by insulin alone treatment.(7)Pearson correlation analysis showed that FFA and BMI(r=0.246,P<0.01),Hb A1c(r=0.259,P<0.01),FPG(r=0.180,P<0.01)in the DM group.(8)Multiple linear regression analysis,in the DM group FFA and BMI(b=0.003,P<0.01),Hb A1c(b=0.016,P<0.01),FPG(b=0.011,P<0.01),TG(b=0.023,P<0.01),HDL-C(b=0.073,P<0.01).Conclusion:(1)The FFA level of the DM group increased,and there was a statistical difference compared with the control group(P<0.01).(2)In the DM group,Pearson correlation analysis showed that FFA level and BMI(r=0.246,P<0.01),multiple linear regression analysis FFA level and BMI(b=0.016,P<0.01);there was no correlation with Hb A1c level and DM Correlation.Therefore,weight loss may be effective in reducing FFA.(3)Among different hypoglycemic drug treatment regimens,insulin+biguanide hypoglycemic regimens have the lowest FFA level compared with other hypoglycemic regimens,followed by insulin treatment alone.
