Effects Of MBCT Combined With The Antidepressant On Symptoms Improvement, Psychological Resilience And Attention Bias In Patients With Depression

Objective: To analyze the effects of mindfulness-based cognitive therapy(MBCT)combined with the antidepressant on symptoms improvement,psychological resilience and attention bias in patients with depression.Methods: A total of 60 patients with depression admitted to open wards of Huainan Fourth People’s Hospital were enrolled between January 2020 and January 2021.According to random number table method,they were divided into observation group and control group,30 cases in each group.The control group was treated with antidepressant(fluvoxamine maleate),while observation group was additionally treated with MBCT.All were treated for 8 weeks.The demographic data such as gender and age were compared between the two groups.The depression symptoms were evaluated by the 17 items of Hamilton Depression Scale(HAMD)before and after treatment.The psychological resilience was evaluated by Connor-Davidson Resilience Scale(CD-RISC)before and after treatment.The mindfulness level was evaluated by Five-factor Mindfulness Questionnaire(FFMQ)before and after treatment.The attention time of emotional words was evaluated by emotional stroop task before and after treatment to assess the attention bias of depression patients.The suicide idea was evaluated by self-rating idea of suicide scale(SIOSS)before and after treatment.The severity of social disability was evaluated by social disability screening schedule(SDSS)before and after treatment.Spearman correlation analysis was used to analyze the relationship between the score reduction rate of HAMD before and after treatment and the difference of each index before and after treatment.Results: After treatment,HAMD scores in both groups were significantly decreased(P<0.05),which were significantly lower in observation group than control group(P<0.05).Before treatment,there was no significant difference in total score of CD-RISC or scores of different dimensions(toughness,self-improvement,optimism)between the two groups(P>0.05).After treatment,the above scores in both groups were significantly increased(P<0.05),which were significantly higher in observation group than control group(P<0.05).Before treatment,there was no significant difference in total score and scores of mindfulness between the two groups(P>0.05).After treatment,scores of observation,description,conscious actions,non-judgment and non-response,and total score of mindfulness in both groups were increased(P<0.05),which were significantly higher in observation group than control group(P<0.05).Before treatment,there was no significant difference in the response time of emotional words between the two groups(P>0.05).After treatment,attention time of negative emotions,neutral emotions and positive emotions in both groups were shortened.The response time of negative emotion words in observation group was significantly shorter than that in control group(P<0.05),but there was no significant difference in response time of neutral emotion and positive emotion words between the two groups(P>0.05).Before treatment,there was no significant difference in SIOSS and SDSS scores between the two groups(P>0.05).After treatment,scores of SIOSS and SDSS in both groups were decreased(P<0.05),which were significantly lower in observation group than control group(P<0.05).The score reduction rate of HAMD before and after treatment was negatively correlated with the total score of CD-RISC,tenacity,self-improvement,optimism,observation,description,conscious action,non-judgment,non-reaction and mindfulness,and positively correlated with negative emotion vocabulary,SIOSS and SDSS before and after HAMD treatment(P<0.05).There was no significant correlation between the score reduction rate and neutral emotion vocabulary and positive emotion vocabulary before and after HAMD treatment(P>0.05).Conclusion: MBCT combined with the antidepressant can significantly relieve depressive symptoms,significantly enhance psychological resilience,improve attention bias,significantly increase mindfulness level,reduce suicide idea and improve social function in depression patients,which can make them return to society faster.The improvement of clinical depression was correlated with psychological elasticity,mindfulness level,suicidal ideation and social function.
