| Objective: Optical coherence tomography angiography(OCTA)was used to observe the changes of blood flow in macular area and optic disc after vitrectomy combined with silicone oil filling in patients with rheotropic retinal detachment.Methods: Thirty patients(30 eyes)with rheogenous retinal detachment admitted to the Ophthalmology Department of the First Affiliated Hospital of Bengbu Medical College from May 2021 to February 2022 underwent vitrectomy combined with silicone oil filling.best corrected visual acuity(BCVA)was measured at 1 week,1month,3 months,and 6 months after surgery.superficial capillary plexus flow density(SCP)and deep capillary plexus flow density(SCP)in macular area were observed by OCTADCP),changes in foveal avascular zone(FAZ),and Disc blood flow density.Results: 1.BCVA: The BCVA of the patients was 1.11±0.20 1 week after surgery,0.66±0.12 1 month after surgery,0.67±0.14 3 months after surgery,and 0.96±0.236 months after surgery.BCVA was increased in 1 month,3 months and 6 months after surgery compared with 1 week,with statistical significance(P < 0.05).There was no significant difference between 3 months and 1 month after surgery(P=0.999).BCVA decreased in 6 months,1 month and 3 months after surgery,with statistical significance(P < 0.05).2.SCP: The SCP of the patient 1 week after surgery was 42.7±7.5,1 month after surgery was 43.4±7.8,3 months after surgery was 45.7±8.0,and 6 months after surgery was 38.3±7.6.There were no significant changes in SCP at 1 month,3months and 6 months after surgery compared with 1 week,respectively,and the differences were not statistically significant(P > 0.05).There were no significant changes in SCP at 6 months and 3 months after surgery compared with 1 month after surgery,with no statistical significance(P > 0.05).6 months after surgery,SCP decreased significantly compared with 3 months after surgery,the difference was statistically significant(P < 0.05).3.DCP: The DCP of the patient was 48.4±5.2 1 week after surgery,45.5±4.7 1 month after surgery,48.0±4.2 3 months after surgery,and 48.3±5.1 6 months after surgery.There were no significant changes in DCP at 1 month,3 months and 6months after surgery compared with 1 week,respectively,and the differences were not statistically significant(P > 0.05).There were no significant changes in DCP at6 months and 3 months after surgery compared with 1 month after surgery,with no statistical significance(P > 0.05).There was no significant difference in DCP between 6 months and 3 months after surgery(P > 0.05).4.FAZ: The FAZ of patients 1 week after surgery was 0.33±0.2,1 month after surgery was 0.32±0.2,3 months after surgery was 0.31±0.1,and 6 months after surgery was 0.31±0.1.There were no significant changes in FAZ at 1 month,3months and 6 months after surgery compared with 1 week,respectively,with no statistical significance(P > 0.05).There were no significant changes in FAZ 6months and 3 months after surgery compared with 1 month after surgery,with no statistical significance(P > 0.05).There was no significant difference in FAZ between 6 months and 3 months after surgery(P > 0.05).5.Optic disc blood density: The blood density of the patient was 42.6±6.1 one week after surgery,42.4±6.0 one month after surgery,41.5±6.2 three months after surgery,and 38.5±6.2 six months after surgery.There were no significant changes in the blood flow density of optic disc 1 month and 3 months after surgery compared with1 week,respectively,with no statistical significance(P > 0.05).The blood flow density of optic disc decreased significantly 6 months after surgery compared with1 week after surgery,with statistical significance(P < 0.05).There was no significant change in blood flow density in optic disc 6 months and 3 months after surgery compared with 1 month after surgery,respectively,with no statistical significance(P > 0.05).Compared with 3 months after surgery,there was no significant change in optic disc blood flow density(P > 0.05).Conclusions: 1.OCTA can observe the changes of retinal blood flow under the filling of silicone oil and present the quantified results,which is convenient,accurate and efficient for observing the changes of retinal blood flow and can become an effective tool for monitoring retinal blood flow.2.No significant changes were observed in blood flow density in macular area and optic disc within 3 months after the vitreous cavity was filled with silicone oil,indicating that silicone oil filling was safe and effective in the short term.3.Six months after the vitreous cavity was filled with silicone oil,BCVA,SCP and optic disc blood flow density all decreased.Long-term silicone oil filling would affect retinal blood flow. |
