Objective:To observe the clinical efficacy of plaster rubbing combined with extracorporeal shock wave in the treatment of early knee osteoarthritis,and to seek a better treatment plan for this disease compared with plaster rubbing and extracorporeal shock wave alone,and to provide a new idea for clinical treatment of this disease.Methods:From November 2021 to December 2022,94 patients in the first Department of Massage and the Fourth Department of Massage in the Second Affiliated Hospital of Heilongjiang University of Chinese Medicine who met the inclusion criteria were numbered according to the time of treatment and divided into three groups by random number table method: plaster massage group,shock wave group and combined group,including 32 patients in plaster massage group and 31 patients in combined shock wave group.The ointment and rubbing group was treated with ointment and rubbing method,with 5 days of treatment and 2 days of rest,and a total of 3 courses of treatment.Shockwave group was treated with shockwave method,and was treated once on the 1st,4th,8th,11 th,15th and 18 th day respectively.The combined group was treated with plaster and rubbing combined with extracorporeal shock wave,and the treatment time was the same as that of plaster and shock wave group.The changes of pain,symptoms and signs before and after treatment in the three groups were observed,recorded and analyzed.Evaluation data were collected to evaluate the clinical efficacy of the three groups.Results:1.VAS score analysisVAS score comparison before and after treatment of the three groups showed statistically significant differences(P < 0.01),indicating that plaster and rubbing method,extracorporeal shock wave method or the combination of both can have a good therapeutic effect on early KOA pain;The VAS score and the number of quarterbacks in the combined group decreased significantly after treatment,suggesting that the combined group was better than the plaster group and extracorporeal shock wave group in relieving early KOA pain2.Comparison of WOMAC scoresBefore and after treatment in all dimensions of the three groups showed statistically significant differences(P < 0.01),indicating that the three groups could significantly relieve the symptoms of early KOA joint pain,joint stiffness,joint mobility and other symptoms.After treatment,the WOMAC pain score of the combined group was significantly different from that of the plaster or shock wave groups(P < 0.05).After treatment,the WOMAC stiffness score of the combination group was significantly different from that of the plaster group(P < 0.05),but there was no significant difference between the combination group and the shock wave group(P > 0.05).After treatment,there was no significant difference in WOMAC joint function score between the combined group and the plaster group(P > 0.05),but there was significant difference between the combined group and the shock wave group(P < 0.05).After treatment,the total score of WOMAC in the combined group was statistically significant compared with that in the plaster or shock wave group(P < 0.05),indicating that the combined group was superior to the plaster or shock wave group in improving early KOA pain and overall curative effect.The combined group improved the early KOA joint function better than the shock wave group,and the effect was comparable to the plaster group.3.SF-12 score analysisOf the three groups before and after treatment in each dimension of SF-12 scale showed significant differences(P < 0.01),indicating that the three groups could significantly improve the physical and mental health of patients with early KOA stiffness.After treatment,MCS score and PCS score of combination group were statistically significant compared with plaster group or shock group(P < 0.05),indicating that combination group was superior to plaster group or shock wave group in improving the mental health of early KOA patients,and plaster group and shock wave group had similar efficacy.4.Effective rate analysisThe total effective rate of the plaster group was 75.0%,the shock wave group was 67.7%,and the combined group was 90.3%.The effective rate of the combined group was higher than that of the plaster and shock wave groups,indicating that the combined group had better clinical efficacy in the treatment of early KOA.Conclusions:1.All three groups of treatment can effectively improve the symptoms of pain,stiffness,joint function and other symptoms of early KOA patients,as well as improve their physical and mental health status.2.The effect of combination group on early KOA was better than that of plaster and shock wave group,and the effective rate of combination group on early KOA was significantly higher than that of plaster and shock wave group.3.The combination of ointment and rubbing combined with extracorporeal shock wave in the treatment of early KOA has obvious effect and high safety,which provides a new idea and a new method for the dialectical treatment and treatment of early KOA in the future,and is worthy of further promotion in clinical practice.
