Clinical Observation On The Treatment Of Diabetic Foot Disease With Platelet-Rich Plasma Combined With Modified Buyang Huanwu Decoction

Objective:In this study,we compared the effects of platelet-rich plasma combined with Modified Buyang Huanwu Decoction with those of platelet-rich plasma alone in the treatment of diabetic foot disease with deficiency of qi and blood and stagnation of ligaments.Methods:Forty patients with diabetic foot disease with deficiency of both qi and blood and stagnation of ligaments and veins who were hospitalized in the Department of Peripheral Vascular Disease of the First Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine from November 2021 to January 2023 and met the inclusion criteria were selected.The patients were randomly divided into treatment group and control group,20 patients in each group,and both groups were treated with medical symptomatic treatment to control the underlying disease and infection,and surgical local debridement treatment to control the trauma,and then the treatment group was treated with platelet-rich plasma combined with Modified Buyang Huanwu Decoction,platelet-rich plasma once every 14 days and Modified Buyang Huanwu Decoction once daily,taken in warm doses in the morning and evening;the control group was treated with platelet-rich plasma alone.The control group was treated with platelet-rich plasma alone,once every 14 days,and both groups were treated for 28 days.The trauma surfaces of all patients were photographed with a digital camera under suitable distance and light conditions,and the trauma surfaces were analyzed using Image J software to obtain the corresponding trauma area values.Blood samples were collected and measured for white blood cell count(WBC)and C-reactive protein(CRP).The TCM evidence score,quantitative trauma score,WBC and CRP values were recorded before and after 28 days of treatment in both groups,respectively.All data were statistically analyzed using SPSS24.0.Results:1.By comparing the trauma area and quantitative trauma score of the two groups before and after treatment,the local trauma condition was better in both the treatment group and the control group than before treatment,and the local trauma improvement in the treatment group was better than that in the control group(P<0.05).2.By comparing the inflammation indexes WBC and CRP between the two groups before and after treatment,the inflammation situation of both the treatment group and the control group was better than that before treatment,and the improvement of traumatic inflammation in the treatment group was better than that in the control group(P<0.05).3.By comparing the total effective rate of Chinese medical evidence and trauma quantification in the two groups before and after treatment,both the treatment group and the control group could effectively treat diabetic foot disease with deficiency of both qi and blood and stagnation of ligaments and improve systemic symptoms,and the treatment modality of the treatment group was better than that of the control group(P<0.05).Conclusion:1.Platelet-rich plasma combined with the Modified Buyang Huanwu Decoction can safely and effectively treat diabetic foot disease of the type with deficiency of both qi and blood and stagnation of ligaments and improve systemic symptoms,and its therapeutic effect is better than that of platelet-rich plasma alone.2.Platelet-rich plasma combined with Modified Buyang Huanwu Decoction can improve the condition of the wound and promote the healing of the wound better than using platelet-rich plasma alone.3.The effect of platelet-rich plasma in combination with Modified Buyang Huanwu Decoction in suppressing the inflammatory reaction of trauma and controlling the inflammation of trauma was better than that of platelet-rich plasma alone.
