Clinical Observation Of Auricular Acupressure Combined With Metformin In The Treatment Of Obese PCOS-IR In The Reproductive Age Group

Objective:In this study,the clinical efficacy of auricular point sticking combined with metformin in the treatment of obese patients with polycystic ovary syndrome(PCOS)-IR in childbearing age was observed by a controlled study of auricular point sticking combined with metformin tablets and metformin tablets alone,provides a basis for clinical auricular acupressure studies in the treatment of PCOS.Methods:In this study,cases were collected from October 2021 to January 2023 from patients attending and recruited at the gynaecology outpatient clinic of the First Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine.80 patients who met the diagnostic criteria of obese PCOS-IR in reproductive age and was identified by Chinese medicine as having heat-damp obstruction in the stomach were included.The patients were divided into 40 cases in the control group and 40 cases in the treatment group using the randomized grouping method.The control group was treated with oral metformin tablets(500mg,taken orally 3 times a day),while the treatment group was treated with auricular acupressure on top of the control group.Both groups were treated for 3 months and patients in both groups were given lifestyle guidance during the treatment period.Before and after treatment,the TCM syndrome score,hirsutism,acne,acanthosis nigrosin,body mass index(BMI),waist hip ratio(WHR),sex hormone levels,fasting plasma glucose(FPG),fasting insulin(FINS),HOMA-IR,ovarian volume and follicle number under ultrasound,basal body temperature(BBT)and other indicators were observed in the two groups.Statistical research results and effect evaluation were made.Results:1.Comprehensive efficacy: The comprehensive efficacy of the treatment group was 86.84%,while the total efficacy of the control group was 67.57%,the comprehensive clinical efficacy of the treatment group was better than that of the control group(P < 0.05).2.Total TCM syndrome score: The TCM syndrome score decreased in both groups after treatment(P < 0.01),and the treatment group was better than the control group when compared between groups(P<0.05).3.Hirsutism,acne and acanthosis nigricant scores: There was no significant change in hirsutism scores after treatment in both groups(P>0.05),acanthosis nigricans scores and acne scores were reduced(P<0.05),and when comparing acne scores between groups,the treatment group was better than the control group(P<0.05).4.BMI and WHR: BMI and WHR values decreased in both groups after treatment(P < 0.01),and the treatment group was better than the control group when comparing between groups(P<0.05).5.Sex hormone levels: LH,LH/FSH and T values decreased in both groups after treatment(P < 0.01),T values were not statistically significant when compared between groups(P > 0.05),LH and LH/FSH values were better in the treatment group than in the control group(P<0.05).6.Insulin resistance indexes: FPG,FINS and HOMA-IR values decreased in both groups after treatment(P< 0.05),and the treatment group was better than the control group when comparing FINS and HOMA-IR between groups(P<0.05).7.Ultrasound indexes: there was no significant change in ovarian volume after treatment in both groups(P>0.05),the number of follicles in the ovaries decreased(P<0.05)and the maximum follicle diameter increased(P<0.05),comparing the maximum follicle diameter between groups,the treatment group was better than the control group(P<0.05).8.BBT: After treatment,the BBT biphasic phase was restored in both groups(P < 0.05),and the BBT biphasic phase was restored more significantly in the treatment group when compared between groups(P <0.05).Conclusions:1.Auricular acupressure combined with metformin can improve the TCM symptoms in obese PCOS-IR patients of reproductive age,effectively restore the menstrual cycle and restore ovulatory function,and the efficacy is better than that of metformin alone.2.Combining auricular acupressure with metformin can effectively improve insulin resistance in obese PCOS-IR patients of reproductive age,effectively reduce body weight,reduce central obesity,restore ovulatory function.3.The overall efficacy of auricular acupressure combined with metformin in the treatment of obese PCOS-IR in reproductive age is better than that of metformin,and the treatment with auricular acupressure is less expensive and has fewer adverse effects,which is worthy of clinical promotion.
