Objective: to explore the influence of shooting dose and image post-processing technology on the quality of DR chest film in pneumoconiosis digital radiography(digital radiography,DR),to provide suggestions on the parameter setting of pneumoconiosis diagnosis chest film taken by all kinds of DR instruments,and to analyze the consistency between display reading and hard copy reading in pneumoconiosis image diagnosis.This provides a basis for improving the quality of DR images and the detection rate and accuracy of pneumoconiosis image screening.Materials and methods: 1.33 different exposure dose combinations of adult chest phantom were taken in a hospital in Hangzhou,and the obtained digital images were objectively detected and subjectively evaluated by computer programming method(Python)and professional doctors to evaluate the quality and noise of DR chest films.2.The chest films of 35 occupational health examination personnel were randomly selected by the radiology department of a hospital in Hangzhou,and the image postprocessing parameters of different groups were set and printed uniformly;the quality of chest films was evaluated by the measurement of optical densitometer and the combination of subjective and objective by professional doctors.3.One hundred and forty-five workers exposed to dust in a coal mine were randomly selected,and the electronic images and printed chest films of the same DR photography examination were carried out in the same hospital in the same week;three intermediate professional title experts qualified for occupational disease diagnosis were selected for display and hard copy reading;the overall density of small shadow,large shadow and the stage of pneumoconiosis were observed,and the consistency of the results of the two reading methods was analyzed.Results: 1.It was found that when the shooting dose was 125KV(kilovolts)and at least5 m As(millisecond),the noise of chest X-ray gradually decreased,and there was significant difference compared with other shooting doses.The qualified rate of pneumoconiosis DR film was also higher.2.It is found that when the density is set to2 and the contrast / detail contrast is 4.5,the optical density of each area of DR chest film meets the requirements of chest X-ray quality,and the qualified rate of physician quality evaluation is the highest.3.The research shows that the results of the four occupational disease diagnosis experts using soft copy and hard copy to diagnose pneumoconiosis are highly consistent(Kappa values are 0.862,0.780,0.60 and 0.683,respectively).The four experts also had moderate and high consistency in the overall intensity of pneumoconiosis diagnosis using soft-copy and hard-copy reading(Kappa values were 0.704,0.644,0.576 and 0.585,respectively).Conclusion: 1.The shooting dose of pneumoconiosis DR chest X-ray should be properly adjusted and the millisecond should be set above 5m As in order to obtain higher quality DR chest X-ray.Second,optimizing the setting of image post-processing parameters can also improve the quality of DR chest radiographs.The parameters suitable for Philips DR for pneumoconiosis imaging diagnosis are density(D)2.0,contrast(gamma,G)and detail contrast(DC)4.5.Other brands of DR instruments can also be tested and adjusted according to the parameters recommended in this study.Third,under the condition of ensuring the consistent and excellent quality of DR images,the diagnosis results of pneumoconiosis diagnosed by occupational disease diagnosis physicians using monitor reading and hard copy reading have moderate or above consistency;it is suggested that soft copy reading should also be included in the professional diagnosis standard of pneumoconiosis.
