Objective:The brain volume and relaxation values of Parkinson’s disease patients were analyzed by 3D-T1MPRAGE high-resolution sequences and Synthetic Magnetic resonance imaging(Sy MRI).To summarize and compare the differences in brain MRI imaging features,nuclear mass structure volume and relaxation value parameters between normal control group and PD(PD)patients,and explore the diagnostic value based on changes in nuclear mass structure for Parkinson’s disease.To observe the changes of nuclear body volume in patients with Parkinson’s disease(PD)and analyze the clinical significance of the differences.Methods:In this study,58patients with Parkinson’s disease were collected as case group,and 46 healthy volunteers were included as control group.All patients underwent 3DT1-MPRAGE high-resolution sequence and synthetic MRI scanning.Free Surfer software analysis tool was used to accurately and automatically segment and analyze subcortical gray matter and each nuclear mass volume.Synthetic MRI was transmitted to MAGi C special post-processing software for all patients and the ROI(region of interest)of nuclear mass was manually delineated.Nuclear mass volume,T1 and T2 relaxation values and proton density Pr D values were obtained,and the differences of the above parameters between PD group and control group were compared to observe whether Parkinson’s disease is related to the changes of the brain kernel mass.Results:Compared with the control group,the volume of the right caudate nucleus(3300±276mm~3vs 3643±459mm~3,P=0.045)and the right accumbens(455±105mm~3vs465±27mm~3,P=0.046)decreased in PD group,and the difference was statistically significant.There was no significant difference in the volume of cerebellar white matter and subcortical gray matter between PD group and control group(P>0.05).T1 relaxation value in bilateral caudate nucleus increased(P<0.05),and Pr D value in left caudate nucleus decreased(P<0.05).Receiver operating characteristic(ROC)curves were plotted respectively,and area under the curve(AUC)was calculated.T1 value of left caudate nucleus was the highest in PD group and HC group,with AUC of 0.902,sensitivity of 0.933 and specificity of 0.850%.Compared with control group,T1,Pr D and T2 values of bilateral putamen were higher than those of control group,with statistical difference(P<0.05).The values of T1,T2 and Pr D in the left thalamus,and the values of T1 and Pr D in the right thalamus were higher than those in the control group,with statistical difference(P<0.05).T1 relaxation values of bilateral substantia nigra and red nucleus were increased in PD group(P<0.05),T2 relaxation values decreased and Pr D values increased in both substantia nigra and red nucleus(P<0.05).T1 value of substantia nigra was positively correlated with the course of disease(r=0.839).The right erythrocyte T1 value had the highest diagnostic efficacy in PD and healthy controls,with an AUC of 0.910,a sensitivity of 85.7%,and a specificity of 90%.Conclusion:In this study,we investigated the value of 3DT1-MPRAGE combined with synthetic MRI in the evaluation of brain structure and relaxation values in patients with Parkinson’s disease.Changes in the volume of the right caudate nucleus,the right nucleus accumbens and relaxation values of the caudate nucleus,substantia nigra,and the red nucleus may provide imaging reference for further clinical evaluation and prediction of PD patients with other symptoms in the future,promote the improvement of the efficacy of MRI in PD diagnosis,and provide objective basis for monitoring the efficacy of patients.
