Objective: To investigate the value of transrectal multimodal ultrasound combined with clinical indicators in the diagnosis of prostate cancer.Methods:A retrospective analysis was performed on 118 clinically suspected patients with prostate cancer admitted to the First Affiliated Hospital of Xinjiang Medical University from August 2021 to December 2022.All patients underwent laboratory index detection and transrectal multimodal ultrasound(transrectal gray scale ultrasound,color Doppler ultrasound,contra-enhanced ultrasound,shear wave elastography).According to the histopathological results of puncture biopsy,the patients were divided into prostate cancer group(51 cases)and non-prostate cancer group(67 cases).The clinical data and ultrasonic characteristics of the two groups were compared.Logistic regression analysis was used to establish regression model A for the diagnosis of prostate cancer with multi-modal ultrasonic indicators.Based on this model,combined with laboratory examination and clinical data,prostate cancer diagnosis model B was established,and(receiveroperatingcurve,ROC)curve was drawn.According to the areaunder ROC curve(AUC),the diagnostic efficacy of the two newly established models,laboratory indicators and clinical characteristics was compared.Results: the age group of patients with prostate cancer,prostate volume,prostate outer gland volume and prostate specific antigen(prostatespecifificantigen,PSA),free prostate specific antigen,the percentage of free prostate specific antigen,and the difference between the prostate cancer group were statistically significant(P<0.05);There were statistically significant differences in internal echo,RI,color Doppler blood flow grading,enhancement pattern and elastic grading(P<0.05),while there was no statistically significant difference in calcification between the two groups(P> 0.05).Multiple Logistic regression showed that:There were statistically significant differences in color Doppler blood flow grading,intracranial vascular RI and enhancement pattern.Regression equation Y=-3025+0.64RI+1.43 enhancement pattern +1.35 blood flow grading for Model A was established.Univariate and multivariate Logistic regression analysis of model A combined with age,PSA and external gland volume showed that there were statistically significant differences in model A,PSA and age(all P<0.05).Regression equation of Model B was established as follows: Y=-4.664+0.837 Model A+0.09 age +0.04 PSA.The area under the curve of model B for diagnosis of prostate cancer was 0.898,sensitivity was 80.0%,specificity was 87.0%,positive predictive value was 83.0%,and negative predictive value was 80.7%.The area under the curve was higher than that of model A,age and PSA,and the differences were statistically significant(all P<0.05).Conclusion: The value of transrectal multimodal ultrasound combined with clinical indicators is higher than that of multimodal ultrasound alone in the diagnosis of prostate cancer.
