Objective: To explore the risk factors and clinical prognostic analysis of dialysis patients with heart failure,so as to provide theoretical basis for early control of risk factors and evaluation of patients’ prognosis.Methods:(1)Regular dialysis patients admitted to the First Affiliated Hospital of Xinjiang Medical University from January 1,2018 to December 31,2020 were collected.381 patients were included.According to the diagnostic criteria of the New York Heart Association,patients with or without heart failure were divided into the group with heart failure(n=178)and the group without heart failure(n=203).Multivariate binary Logistic regression analysis was used to determine independent risk factors for dialysis patients with heart failure.(2)381 dialysis patients were followed up.The death outcome events of patients were recorded during the follow-up,and COX regression analysis was performed to determine the independent risk factors affecting dialysis patients’ death.Results:(1)Patients were grouped according to whether they had combined heart failure,After unifactor analysis of the two groups,complications(hypertension,diabetes,coronary heart disease),smoking history,left ventricular hypertrophy,age,left ventricular ejection fraction,NT-pro BNP,left ventricular diastolic diameter,left ventricular systolic diameter,E/A,glycohemoglobin,low density lipoprotein,troponin,creatine kinase MB isoenzyme,C The difference of reactive protein was statistically significant between the two groups(P<0.05).Multivariate binary Logistic regression analysis showed that comorbidities(hypertension,diabetes,coronary heart disease),smoking history,NT-pro BNP were independent risk factors for dialysis patients with heart failure.(2)During follow-up,COX regression analysis showed that age and NT-pro BNP were independent risk factors for death outcomes in dialysis patients.Conclusion:(1)In dialysis patients with heart failure,comorbidities(hypertension,diabetes,coronary heart disease),smoking history,NT-pro BNP are independent risk factors for dialysis patients with heart failure.(2)During follow-up,cardiovascular mortality was higher in the group with heart failure,and age and NT-pro BNP were associated with increased risk of death in dialysis patients.(3)NT-pro BNP is not only an independent risk factor for heart failure in dialysis patients,but also associated with an increased risk of death in dialysis patients. |
