PurposeRenal cell carcinoma is a common urogenital malignancy,and the number of patients diagnosed in China is increasing year by year.In recent years,some studies have found that the early diagnosis and accurate staging of RCC are crucial for optimizing the subsequent treatment method.In this study,we analyzed the variability of CD70expression between RCC patients and normal patients to explore the diagnosis of CD70,and the correlation with clinicopathological features on RCC.MethodsThe clinical data and pathological data of RCC patients treated in the First Affiliated Hospital of Bengbu Medical College from January 2019 to December 2022 were collected,including sex,age and survival;the pathological data included pathological classification,tumor size,presence of lymph node metastasis,degree of tumor differentiation,renal vein infiltration,tumor necrosis,distant metastatic status,VHL,PD-L1,cytotoxic T cells,regulatory T cells,M2 macrophages,and FAP+CAF expression.After further screening of the inclusion criteria and exclusion criteria,a total of 113 patients were selected for the RCC group,and another 30 normal renal tissues were included in the control group.For the clinical characteristics of CD70 expression in RCC patients and their associated risk factors,the independent risk factors for CD70expression in RCC patients were analyzed by multivariate Logistic regression analysis.Correlation between CD70 expression and clinicopathological features was calculated by independent t-test and Pearson’s chi-square(χ~2)test.Tumor CD70 was used for the expression of immune-related cell markers in the tumor microenvironment..ResultsAmong the 113 RCC patients,the positive rates of CD70 expression of renal clear cell carcinoma,papillary kidney carcinoma and chromophobe carcinoma were 87.01%,43.75%and 20.00%,respectively,and clear cell carcinoma CD70 expression was higher than papillary carcinoma and chromophobe carcinoma(p<0.05).Among them,CD70 expression in clear cell carcinoma was higher in poorly differentiated than highly differentiated kidney carcinoma(p<0.05),and the positive rate were 88.24%,86.05%and 75.00%in highly differentiated,poorly differentiated and 75.00%,respectively.Among poorly differentiated renal clear cell carcinoma,the highest high expression rate of CD70 reached 55.81%,and 50.00%in renal cancer metastases,and the least high expression rate was 41.18%in highly differentiated,well-differentiated renal cancers.The expression of CD70 was closely related to patient sex,tumor size,tumor differentiation,renal vein infiltration,tumor necrosis,and distant metastasis status(p<0.05).The expression of CD70 was higher in patients with poor survival prognosis(p<0.05).Multivariate regression analysis identified gender(OR=3.568),tumor size(OR=3.005),tumor necrosis(OR=3.717)and distant tumor metastasis(OR=10.805)were independent risk factors for CD70 expression in RCC patients(p<0.05).The expression of CD70 in the tumor microenvironment was positively correlated with PD-L1 expression and the infiltration of cytotoxic T cells and in FAP+CAF(p<0.05).ConclusionsRCC tissue showed a high rate of positive CD70 expression,in which renal clear cell carcinoma CD70 expression was related to patient sex,tumor size,tumor differentiation,renal vein infiltration,tumor necrosis,distant metastasis,and the survival status of patients.CD70 expression was high in high differentiated,poorly differentiated and metastatic lesions,which is a potential predictive indicator of renal clear cell carcinoma.At the same time,CD70 is positively correlated with PD-L1 expression,cytotoxic T cells and FAP+CAF infiltration.Visualization and monitoring of CD70 expression in RCC can not only help in the diagnosis of renal cell carcinoma,but also help to guide the targeted treatment of CD70.
