Objective:The outpatient medical records of Professor Li Ji in the treatment of depression were collected,and the data mining processing technology was used to analyze the data.The medication rules and characteristics of Professor Li Ji in the treatment of depression by differentiation were summarized and analyzed,and his clinical experience and academic thought in the treatment of depression by differentiation were summarized.Methods:The outpatient medical records of patients with depression treated by Professor Li Ji from January 2017 to December 2022 were collected,and 102 medical records(254consultations in total)that met the research criteria were screened.The basic information of outpatient medical records,including sex name,gender,age,four diagnosis and medication prescription,was standardized,and a database was established.Using WPS Excel 2019 and "Traditional Chinese Medicine Inheritance Computing platform V3.0" software,the database data were analyzed by gender,age,symptoms,drug frequency,efficacy,nature and taste,association rules and cluster analysis,etc.To explore the medication rules of Professor Li Ji in the treatment of depression,and summarize the academic thought of depression treatment.Results:1.A total of 102 medical records were included in this study,including 20 male patients,accounting for 19.61%;There were 82 female patients,accounting for 80.39%.The average age of the patients was 48.76 ± 11.18 years old,and the patients over 30 years old accounted for 93.14% of the total,mainly between 40 and 59 years old,accounting for62.74% of the total.The course of disease was less than 1 year in 30.39%,1-5 years in28.43%,less than 5 years in 58.82%,and more than 6 years in 20.58%.In terms of past medical history,digestive system diseases accounted for the largest proportion,accounting for 33.87%,gastritis,cholecystitis,fatty liver were more common,followed by circulatory system diseases,accounting for 20.97%.2.In terms of symptoms,the most frequent symptoms were insomnia,anger,fatigue,loose stool,chest tightness,shortness of breath,loss of appetite,palpitations,dreams,abdominal distension,etc.The number of support was set as 50,and the confidence was 0.7.The top three symptoms were shortness of breath → chest tightness,dreaminess →insomnia,and good anxiety → insomnia.There were five main syndrome types: deficiency of both heart and spleen,liver-stagnation and spleen-deficiency,liver-qi stagnation,liver-gallbladder damp-heat and kidney deficiency and liver-stagnation.The frequency of deficiency of both heart and spleen was 40.94%,and that of liver-stagnation and spleen-deficiency was 39.37%.3.A total of 254 prescriptions,a total of 144 Chinese herbs,and a total of 3406 times were included in the study.There were 62 Chinese herbs used more than 10 times,with a total frequency of 3161 times,accounting for 92.81% of the total frequency.Among them,the most frequently used drugs were fried licorice,fried Atractylodes rhizoma with bran,Radix paeoniae alba,Radix bupleuri,ginseng,fried jujube kernel,Poria cocos,Chuanxiong,Tangerine peel,sand kernel,etc.4.In terms of the distribution of drug efficacy categories,there were 16 categories of drug efficacy categories,with a total frequency of 2875 times.The frequency of tonic drugs was the highest,up to 924 times,accounting for 32.14%.The top 10 efficacy categories of drugs used frequently were: tonifying deficiency,regulating qi,clearing heat,promoting blood circulation and removing blood stasis,relieving surface,reducing water and dampness,calming mind,removing dampness,relieving cough and asthma,and relieving astringent.Among the tonifying deficiency drugs,the most was tonifying qi.5.In terms of the statistics of the four qi,five tastes and meridians,the four qi were mainly cold,flat and warm,and the five tastes were mainly sweet and bitter,and they mainly belonged to the heart,spleen,liver,lung and stomach meridians.6.In the aspect of syndrome and drug association rules,for the analysis of syndrome and drug association rules of liver depression and spleen deficiency,the number of support was set as 40 and the degree of support was 0.8.A total of 22 drug patterns and 26 association rules were obtained.Among them,the top 3 drug combinations were bupleurum bupleurum and Paeony;Bupleurum bupleurum,white art with bran;White peony,bran stir-fried white fungus.The top 3 combinations with the highest degree of support were bupleurum,paeony,stir-fried atractylodes atractylodes with bran;Bupleurum,Paeoniae alba,Ligusticum Chuanxiong;Bupleurum,Paeony,tangerine peel.The top 3 association rules of the two drugs with the highest confidence were: Paeony → Bupleurum;Fructus aurantii →Bupleurum;Tangerine peel → Bupleurum.The association rules of the top 3 drugs with confidence were: Paeony,Fructus aurantii → Bupleurum;White peony,bran fried white art→ Bupleurum;White peony,Tangerine peel → Bupleurum.For the analysis of association rules of heart-spleen deficiency syndrome-drug,the number of support was set as 30 and the degree of support was 0.7,32 drug patterns and 27 association rules were obtained.Among them,the top 3 combinations in terms of support were: Stir-fried atractylodes atractylodes with bran and ginseng;Saute sour jujube kernel,cypress seed kernel;Ginseng,astragalus.The top 3 combinations with the highest support were: Stir-fried white Rhizoma atractyloides with bran,ginseng and Astragalus;Stir-fried white fungus,ginseng,poria cocos with bran;Stir-fried white art with bran,ginseng and sour jujube kernel.The top 3 association rules of the two drugs were: Poria cocos → fried Atractylotein with bran;Bupleurum → Paeony;Raw keel → sour jujube kernel.The top three association rules with the highest confidence were: ginseng,poria cocos → white Rhizoma atractyloides stir-fried with bran;Baizi kernel,raw keel → fried sour jujube kernel;Baizi kernel,Astragalus → ginseng.7.Cluster analysis of therapeutic drugs obtained a total of 6 groups of drug core combinations that were more in line with the actual clinical use of the tutor :C1: prepared licorice,bupleurum bupleurum,paeony,ligusticum Chuanxiong,Chen peel,stir-fried white Rhizoma atractyloides with bran,stir-fried sour jujube kernel and amata kernel;C2:Prepared licorice,poria,Fructus aurantii,Coix seed,Bupleurum,Stir-fried white Rhizoma,white Peony,ginseng;C3: Processed licorice,fried white Rhizome with bran,white peony,poria,tangerine peel,sand kernel,Bupleurum,yam;C4: Processed licorice,fried white fungus with bran,ginseng,fried sour jujube kernel,bupleurum,white peony,poria cocos,Fructus aurantii;C5: Roasted licorice,ginseng,ligusticum Chuanxiong,stir-fried white Rhizoma,Salviorrhiza miltiorrhiza,Baizi kernel,Astragalus,stir-fried sour jujube kernel;C6:Prepared licorice,Bupleurum,Stir-fried white Rhizome with bran,white Peony Root,Ophiopogonis ophiopogon,Chinese sand ginseng,Tangerine peel,sand kernel.Conclusion:1.Professor Li Ji’s treatment of depression syndrome differentiation and treatment,exploring the root and source,with the main treatment principles of nourishing the heart and calming the mind,suppressing wood and supporting soil,while based on the whole,reinforcing deficiency and reducing excess,balancing Yin and Yang,taking into account the five viscera,and multi-viscera homology.2.Based on the traditional Chinese medicine inheritance computing platform V3.0,it can be seen that Professor Li Ji is good at stir-fried Baizhu,paeony,bupleurum,ginseng,stir-fried jujube kernel,poria,Chuanxiong,orange peel,amomum kernel and other drugs for the treatment of depression,which can be used as the core drugs for the treatment of depression;The main drugs used were tonifying deficiency,regulating qi,clearing heat,promoting blood circulation and removing blood stasis.3.Based on the traditional Chinese medicine inheritance computing platform V3.0,it can be seen that Professor Li Ji used a combination of warm and cold drugs in the treatment of depression,combining sweetness and bitterness,and the characteristics and tastes were mainly warm,flat,cold,sweet and bitter;The main methods of traditional Chinese medicine are heart channel,spleen channel,liver channel,lung channel and stomach channel. |
