Objective: 1.To explore the effects of Micronized Acellular Dermal Matrix(m ADM)on volume retention,inflammatory infiltration and the number of blood vessels after granular fat injection and transplantation.2.To explore the optimal ratio of acellular dermal matrix microparticles and granular fat combined transplantation.Methods: From a number of rats,groin fat on both sides was extracted,cleaned and cut,coarse fibrous tissue was selected and mixed with acellular dermal matrix particles(m ADM)in different proportions,and 15 male Wistar rats were injected with different fillers on both sides of the back.Pure granular fat(control group C1),pure acellular dermal matrix particles(m ADM)(control group C2),granular fat and acellular dermal matrix particles(m ADM)1:1 mixture(experimental group E1),granular fat and acellular dermal matrix particles m ADM(2: 1)Mixture(experimental group E2),mixture of granular fat and acellular dermal matrix particles(m ADM)4:1(experimental group E3),mixture of granular fat and acellular dermal matrix particles(m ADM)6:1(experimental group E4),0.5ml was injected at each point.Five samples were randomly selected at 4 weeks,8 weeks and 12 weeks respectively to detect the volume of the graft,calculate the volume retention rate,and perform HE and Masson staining to observe the inflammatory infiltration and the number of collagen,and CD34 immunohistochemical staining to observe the graft neovascularization.Results: Six groups of grafts were injected subcutaneously into rats at 4weeks,8 weeks and 12 weeks,respectively.The volume of grafts was measured by drainage method.It was found that,with the passage of time,the simple granular fat group(group C1)and m ADM: granular fat 1: The graft volume of group 4(group E4)had a downward trend,while that of the simple m ADM group(group C2)had a gradual increase,and the graft volume of the other groups had a slight increase and then a decrease,which was statistically significant(P < 0.05).In addition,at multiple time points after transplantation,the graft volume of pure granular fat group(group C1)was the minimum.In the experimental group,the graft volume of m ADM:granular fat 1:1 group(group E1)was the maximum,and that of m ADM: granular fat1:6 group(group E4)was the minimum,with statistical significance(P < 0.05).The results of histological staining indicated that the volume retention rate was the highest after 1:1 mixing of the two groups(E1 group).Histological analysis showed that there was no obvious inflammatory cell infiltration in the m ADM group(group C2)at the early stage,and the inflammatory reaction gradually occurred after 8 weeks.In the granule fat group(group C1),there was vacuole formation and obvious inflammatory cell infiltration.In the experimental group,the inflammatory reaction was the least after 1:1 mixture of the two groups(group E1).The larger the proportion of m ADM and granular fat,the more collagen fibers and the more regular the arrangement.Immunohistochemical results of CD34 showed that the number of graft neovascularization was the highest in granular fat and m ADM1:1 group(E1 group),and the lowest in m ADM: granular fat 1:6 group(E4 group)at the three time points.The number of graft neovascularization in granular fat group(group C1)and experimental group(group E1-E4)was the highest at week 4,and gradually decreased with time.The number of graft neovascularization in the m ADM group(Group C2)was the highest at week 4,but stabilized after that with little change.There was statistical difference in the count of microvessels among all groups at the 4th week(P< 0.05),but no statistical difference at the 8th and 12 th week(P > 0.05).Conclusion: Subcutaneous injection of acellular dermal matrix particles(m ADM)combined with granular fat can effectively maintain the volume of the injected substance for a long time,and the best effect is the 1:1 mixture of the two.
